Zoon's balanitis (balanitis circumscripta plasmacellularis)

Abstract: The clinical and histopathological features of nineteen cases of Zoon's balanitis and a comparable lesion of the vulva have been reviewed. The condition is particularly distinguished by characteristic histological changes affecting the epidermis and the dermal vasculature. The presence of a plasma cell infiltrate is relatively non-specific. Recognition of these features should allow simple differentiation from premalignant, infective and other inflammatory penile lesions which may be more responsive to treatme… Show more

“…Subsequently similar lesions have been described on the vulva23 and the lips.4 This rare disorder consists of a single, red, shiny and smooth patch involving the glans penis or adjacent prepuce or both.' [5][6][7] The characteristic histological features are: a band-like mainly plasmacytic inflammatory infiltrate of the upper dermis, dilated capillaries and deposits of haemosiderin.' 5Treatment with topical steroids8 is palliative at best, with many patients being totally unresponsive.679 Circumcision which was advocated as the best modality of treatment many decades ago has been reported to be beneficial in the recent reports6 7 10 We report 32 cases with this condition 27 of whom were treated successfully with circumcision.…”

“…This rare disorder seen almost exclusively in uncircumcised men has been described as ‘balanoposthite chronique circonscrite bénigne à plasmocytes’ by Zoon in 1952 [2]. According to Souteyrand et al [3], it manifests itself with a solitary asymptomatic circumscribed shiny glazed orange-coloured macular erythema with a maximun diameter of 3 cm. Histology shows a thinned and flattened epidermis and a dense band-like infiltrate in the upper dermis, in which plasma cells usually exceed 50% of the cells present.…”

“…Histologically, epidermal edema, a dense upper dermal band of chronic inflammatory cells, including many plasma cells, dilated capillaries, extravasated red blood cells, and hemosiderin deposition, is observed [23]. However, it has been suggested that the presence of plasma cell infiltrate is relatively not specific for the diagnosis of PCVZ [22,24]. Recently, reflectance confocal microscopy, an in vivo imaging method to get morphological information about architecture and single cells in the skin, was shown to be helpful in differentiating between balanitis and carcinoma in situ and avoiding biopsies at this sensitive site [25].…”
