A key ingredient in the current proposal of the DSM-5 Work Group on Personality and Personality Disorders is the assessment of overall severity of impairment in personality functioning: the Levels of Personality Functioning Scale (LPFS). The aim of this article is to contribute a conceptual and empirical discussion of the LPFS from the perspective of the Operationalized Psychodynamic Diagnosis (OPD) system (OPD Task Force, 2008 ). First, we introduce the OPD Levels of Structural Integration Axis (OPD-LSIA), a measure of individual differences in severity of personality dysfunction that is rooted in psychodynamic theory. We show that the OPD-LSIA is reliable, valid, and highly associated with observer ratings of personality disorders. In the second part, we present results from an OPD expert consensus study exploring potential limitations of the current LPFS item set from the perspective of the OPD-LSIA. We conclude with highlighting implications for future revisions of the DSM-5 proposal.

Zusammenfassung Hintergrund Bei verschiedenen psychischen Störungsbildern, wie beispielsweise Essstörungen, zeigen sich Einschränkungen in der psychischen Struktur, die sowohl bei Erwachsenen als auch Adoleszenten beschrieben wurden. Bisher liegen jedoch keine empirischen Untersuchungen zur psychischen Struktur bei Adoleszenten bzw. jungen Erwachsenen mit typischen oder subklinischen Formen von Essstörungen vor. Ziele der Arbeit Die vorliegende Studie zielt auf die Untersuchung und den Vergleich verschiedener Strukturdimensionen gemäß Operationalisierter Psychodynamischer Diagnostik bei weiblichen Adoleszenten und jungen Erwachsenen mit unterschiedlicher Ausprägung selbstberichteter anorektischer Essstörungssymptomatik ab. Material und Methoden Es gingen Daten von 141 weiblichen Adoleszenten und jungen Erwachsenen mit keiner, gering oder ausgeprägter selbstberichteter anorektischer Essstörungssymptomatik ein. Die Probandinnen wurden über die sozialen Netzwerke Facebook und Instagram rekrutiert und mithilfe einer Onlineerhebung befragt. Zum Einsatz kamen der Fragebogen zur Strukturachse der Operationalisierten Psychodynamischen Diagnostik (OPD-SF) sowie das Eating Disorder Inventory‑2 (EDI-2). Ergebnisse Adoleszente und junge Erwachsene mit ausgeprägter selbstberichteter anorektischer Essstörungssymptomatik zeigen auf fast allen Skalen des OPD-SF sowie auf sämtlichen Skalen des EDI‑2 signifikant höhere Werte als Adoleszente und junge Erwachsene ohne bzw. mit geringer Symptomatik. Diese Unterschiede bleiben auch nach Kontrolle für das Alter sowie Vorliegen von Vordiagnosen und psychotherapeutischen (Vor)Behandlungen bestehen. Es zeigen sich deutliche Zusammenhänge zwischen den Skalen des OPD-SF und denen des EDI‑2. Schlussfolgerung Einschränkungen in der psychischen Struktur scheinen sich bereits bei subklinischen Formen gestörten Essverhaltens zu zeigen, zumindest, wenn diese selbstberichtet ist. Die Ergebnisse unterstützen die Notwendigkeit der frühzeitigen Erkennung von Problemen mit dem Essverhalten und des Einbezugs struktureller Aspekte in die psychotherapeutische Behandlung.

Follow-up evaluation of the stability of symptom reduction in a 5-year follow-up of inpatients aged 60+ yearsObjectives: To date, there are no data available on the long-term follow-up of inpatient psychotherapeutic treatment of elderly patients. Similarly, the stability of therapeutic outcome has not been sufficiently analysed. Age-related diseases are risk factors for mental stability. The question is: How stable are the therapeutic treatment results of patients 60+ years old 5 years after having received hospital treatment? Which is long-term predictive value of OPD ratings for the reduction of symptoms? Methods: In a 1-year follow-up carried out in 2009, 46 patients were assessed regarding their current symptoms, further treatment, and their retrospective judgement of their hospital treatment. Four years later they were then reassessed. Psy-BaDo, SCL-90-R, and clinical diagnoses were recorded at the time of hospital admission and discharge. Again, the same specific follow-up questionnaire was sent to the patients, together with the Brief Symptom Inventory (BSI). An OPD rating was carried out during hospital treatment by two trained OPD raters. To ensure the representativeness of the sample, we compared them with those who answered after only a year of treatment. Results: Twenty-six patients answered 5 years after treatment. 35 % of the patients assessed their hospital treatment as very helpful, 31 % as rather helpful and 31 % as slightly helpful (not helpful 3 %). The reduction in the symptoms assessed with the GSI at the time of discharge proved to be stable at both follow-ups. No patients were found to have low structural integration in the OPD rating. Conclusion:The follow-up of inpatient treatment showed encouraging results for the psychotherapy of elderly patients, also with respect to the long-term effects over a 5-year period. There were no significant correlations between the OPD rating and the severity of symptoms.
