臨床看護における「優先順位」の概念分析

Abstract

Objective:The purpose of this study was to analyze the concept of "priority" in clinical nursing and clarify its definition.Methods: A total of 47 papers were analyzed using Rodgers's (2000) concept analysis approach. Results:The following three attributes were extracted: [estimated from the impact on life], [weighted by the patient's condition], and [variable depending on the situation]. In addition, four antecedents were extracted: [responsibilities both as a nurse and member of the organization], [differences in nurse' proficiency], [influence of organizational culture and systems], and [time constraints and lack of staff]. Finally, three consequents were extracted: [work is performed efficiently], [work is adjusted in order], and [dilemma arises in care delivery].Conclusion: "Priority" in clinical nursing was defined as "The positioning of nursing practice in which the impact on life is estimated, weighted according to the patient's condition to determine their relative importance, and fluctuates according to the situation." 要 旨 目的：本研究の目的は，臨床看護における「優先順位」について概念を分析し，定義を明確にするこ とであった． 方法：本研究では Rodgers(2000)の概念分析のアプローチを用い，47 文献を分析した． 結果：3 つの属性， 【生命への影響から程度が見積もられる】 【患者の状態で重みづけられる】 【状況に よって変動する】 ，4 つの先行要件【看護師および組織の一員としての責務がある】 【看護師の熟練度の 差がある】 【組織文化と制度の影響がある】 【時間の制約や人員の不足がある】 ，3 つの帰結【業務が順当 に調整される】 【効率的に業務が遂行される】 【ケア提供にジレンマが生じる】が抽出された． 結論：臨床看護における「優先順位」を， 「生命への影響度を見積もり，患者の状態によって重みを与 えて相対的に重要度が判断され，状況に応じて変動する看護実践の位置づけ」と定義した． 受付日：2023 年 1 月 24 日 受理日：2023 年 5 月 25 日 1) 同志社女子大学大学院看護学研究科看護学専攻博士課程(後期) Doctoral program, Graduate
