The properties of biocompatible orthopaedic polymer developed as an alternative to the metallic materials used in reconstructive bone surgery are discussed. Experiments were conducted to enhance the mechanical characteristics of the polymer by incorporation of various fibres. The result was a super biocompatible orthopaedic polymer which could be a valuable alternative to intra-medullary long bone metallic rods, whilst normal biocompatible orthopaedic polymer is currently used for bone filling and reconstructive surgery.
The aim of the study was to evaluate bone metabolism in cancer patients with bisphosphonate osteonecrosis of the jaws. The study included 45 people of the main group (patients with cancer with osteonecrosis of the jaw in patients receiving bisphosphonates) and 25 in the control group (cancer patients treated with bisphosphonates, but did not have osteonecrosis of the jaw), which had a stabilization of the underlying disease. Bone metabolism was evaluated by the level of osteocalcin (OC), bone-specific alkaline phosphatase (bALP), aminoterminal of propeptide of procollagen type I (P1NP), tartrate-resistant acid phosphatase (TRAP5b), calcium (Cа), phosphorus (P) in blood serum before treatment and after 6 months. Compared to the average levels of marker patients of the main and control groups using the Mann-Whitney test for p < 0,05. A baseline level of СТХ (0,23±0,02 ng/ml) and OС (11,58±0,54 ng/ml) in the treatment group was significantly (p < 0.05) lower than control group (0,43±0,01 ng/ml and 17,94±0,83 ng/ml), and the level of osteocalcin in the main group (11,58±0,54 ng/ml) was on average below normal 2,59 times. Recorded significantly higher (p < 0,05) levels bALP (133,24±14,03 U/l) and TRAP5b (3,54±0,38 U/l) in patients with osteonecrosis compared with a control group (73,32±3,41 U/l and 3,12±0,12 U/l). Reliably detected differences in the levels of P1NP, Ca and P were not detected (p > 0,05). In the main group after 6 months of treatment was observed a tendency of growth of СТХ, TRAP5b, OK, bALP, P1NP, Ca, but only for the markers of resorption and СТХ, TRAP5b these differences were significant. Indicators of patients in the control group were stable and did not differ in the dynamics. The development of bisphosphonate osteonecrosis of the jaws is directly related to bone metabolism and occurs with predominance of the processes of bone resorption and inhibition of bone formation processes.
The efficiency of photodynamic therapy (PDT) using 5-aminolevulinic acid (5-ALA) is essentially determined by an optimal choice of sensitization means, anong which the application method is one of the most perspective due to high permeability of a skin for 5-ALA.The aim of present work is optimization of sensitization process with ALA. We have used (lie possibility of 5-ALA transparency in applicator (outside of tissue) while protoporphyriii-IX, formed in sensitized tissue, has sufficient absorption for providing PDT. We have developed 5-ALA containing applicator on the basis of transparent biodegradable polymers, which serve as a programmed source of 5-ALA in the zone of their contact with pathological tissue.The investigations carried out on mice with the solid form of leucosis p-388, have shown that developed applicator provides effective sensitization of pathological tissue, supporting high concentration of photosensitiser in tissue during the whole time of application (up to week). The experiments in vivo have demonstrated high efficiency of PDT. The preliminary clinical researches allow to draw a conclusion about its persPectivitY for photodynamic treatment of skin malignancies.
Post-resection wound healing, oral microbiota composition and saliva lysozyme activity were assessed in 36 patients with tongue carcinoma before and after surgery with the local application of 2% solution of acid-diluted chitosan in the postoperative period. The microbiocenosis of the oral cavity before treatment in cancer patients was represented by conditionally pathogenic and pathogenic bacteria, whose number is more than 4 lg CFU/cm, indicating dysbiosis; the lysozyme level was reduced, accounting for 25.4±8.5 µg/ml. After the radical hemi-glossectomy the reduction in the frequency and quantity of conditionally pathogenic and pathogenic microorganisms was statistically determined, as well as an increase in the level of lysozyme of saliva on the 10 day of treatment. Patients receiving local treatment with chitosan showed an increasing number of normal microbiota in 1,5-2 times, and the level of lysozyme in 8 times (amounted to 202.87±56.85 µg/ml).
Introduction. The outbreak of a new coronavirus infection has become a challenge for the global health system. The COVID-19 infection is directly related to various disorders of the cardiovascular system, including the microcirculatory bed, caused by thrombotic events and deteriorations of blood rheology. Aims. The paper reports on the results of a study of Doppler sonographic parameters changes in patients with a novel coronavirus infection over the past 6 months. Materials and methods. We assessed the oral mucosa microcirculation in three segments using the high-frequency ultrasound dopple-rography. Results. We recorded the linear and volumetric blood flow rates and the Gosling and Purselo indexes in the course of our work. When comparing the obtained average statistical parameters of blood flow velocity, the linear and volumetric blood flow rates in patients of both groups were found to be lain in the same range and the mean values of Vas, Vam, Qas were equal. The mean values of the Purselo resistance index were closer to 1,0 in patients with COVID-19, and the values of the Gosling pulsation index (PI) were on average 53.3 % higher than in the control group. Conclusion. We evaluated the screening capabilities and potential of high-frequency ultrasound dopplerography for use in patients of different age groups and different somatic status.
