Внепеченочные хирургические вмешательства у пациентов с циррозом печени ограничивает значительная частота осложнений и летальности. Современные изменения в периоперационном ведении пациентов, использование адаптированных шкал для оценки операционного риска, изменение хирургической техники позволили улучшить результаты лечения и расширить его возможности при многих заболеваниях. Рак толстой и прямой кишки занимает лидирующее положение в структуре онкологической патологии, и хирургия остается основным способом лечения этих заболеваний. Оценка периоперационных рисков у такой сложной категории пациентов позволяет проводить радикальное онкологически обоснованное лечение, которое ранее было им малодоступно. Ключевые слова: цирроз печени, колоректальный рак, аденокарцинома толстой кишки, осложнения, онкология, осложнения цирроза печени.Extrahepatic surgery on the patients presenting with liver cirrhosis has limitations attributable to the high frequency of postoperative complications and lethality. The possibilities for the surgical treatment of extrahepatic pathologies have recently been extended and its outcomes improved by virtue of novel approaches to the perioperative management, the use of adapted scales for the evaluation of the surgical risks, and modification of the operative techniques. Colorectal cancer prevails in the structure of onclological diseases of the gastrointestinal tract and the surgical intervention remains the main method for the management of the patients with this pathology. The adequate assessment of the perioperative risks for such patients creates the prerequisites for the well-substantiated surgical treatment that has recently been unavailable to them.
