A 60-year-old woman presented with new numbness of her right arm and face on awakening; no other neurological signs or symptoms were present. Her past medical history was unremarkable aside from smoking (30 pack years). In addition to a small focus of restricted diffusion, brain MRI showed multiple ''cystic" lesions in both hemispheres (Fig. 1).
