Purpose Traditionally, the ''Halstead block'' has been widely used to provide anesthesia in mandibular teeth. Two other techniques, the Gow Gates mandibular nerve block and the Akinosi Vazirani closed-mouth mandibular nerve block, are reliable alternatives to the conventional inferior alveolar nerve block. The purpose of this study is to evaluate the onset of anesthesia, anesthetic success and incidence of positive aspiration during administration of local anesthetic solution using the Halstead, Vazirani Akinosi and Gow Gates techniques. Materials and method The study involves 210 subjects, divided into three different groups of 70 subjects each receiving Gow Gates, Vazirani Akinosi and conventional inferior alveolar nerve blocks. The onset of anesthesia, positive aspiration and anesthetic success was evaluated. Results In Vazirani Akinosi technique group, patients showed highest anesthetic success of 95.71%; there was a significant difference seen between the Gow Gates and Vazirani Akinosi techniques (p = 0.0241*). The mean value of the onset of anesthesia in Gow Gates technique showed the longest 343.71 ± 153.20 s, in Halstead technique it was 177.43 ± 59.94 s, and in Vazirani Akinosi technique it was 192.86 ± 61.20 s. There was a significant difference seen between Gow Gates and Vazirani Akinosi techniques (p = 0.0001*) and Gow Gates and inferior alveolar nerve block techniques (p = 0.0001*). Conclusion The Vazirani Akinosi technique was found to be significantly better than the other two techniques with respect to both onset and success of anesthesia. Positive aspirations were slightly higher in the conventional IANB technique compared to the other two, but did not reach statistical significance.
Dental implants are the modern marvel and are widely accepted as a reconstructive treatment modality for tooth replacement. In recent times, there has been a marked progress in the clinical success rates of dental implants, but implant failures as a result of infections are continuing at an alarming rate of 8% per year, translating into 1 million failures worldwide. Perimucositis and peri-implantitis are the chief complications reported postimplant surgery that effects its short- and long-term success. Peri-implantitis is characterized by clinical and radiological bone loss around the implant accompanied with an inflammatory reaction of the peri-implant mucosa and is an irreversible condition, whereas perimucositis is a reversible inflammatory change. Implant surfaces provide an ideal substrate for bacterial adhesion forming a biofilm. Biofilm performs vast functions ranging from physical defensive barrier against phagocytic predation to working as a selective permeable barrier. This limits the diffusion of systemic antimicrobial agents that are capable of damaging the bacterial complexes. These rapidly growing bacteria give rise to a chronic infection which is difficult to eradicate by conventional antibiotic therapy. To inhibit peri-implant infections, various functional modifications in the implant surfaces have been suggested. The coatings on the titanium implant are incorporated with disinfectants, antibiotics as well as antimicrobial peptides AMPs. This paper is an attempt to review all the antibiotic coatings available for a titanium implant and discuss their prospective future to prevent peri-implant infections. All the results indicate that microwave glazing can be a feasible option for glazing porcelain specimens. It was concluded that surface topography is influenced by surface treatment and microwave glazed ceramic is superior to conventional oven glazed ceramic and hand-polishing showed greater surface roughness when compared to glazing. IPS CLASSIC ceramic showed relatively smooth surface when compared to VITA VMK MASTER irrespective of the surface treatment.
Before starting with the article, the researcher wishes to express her thoughts relating to the title above. How does it feel to be punished for something which is not even our fault? In spite of living in a democratic nation where we are free to make choices of our life why there is a constant pressure upon us to live as per the terms and conditions of the society merely because of their constant habit of intruding and judging us on the basis of our sexual orientation and gender identity? Why do we have to hide our orientation and gender identity just because we have to fit into the stereotypical norms of the society? Section 377 of the Indian Penal Code, 1860 was a British colonial legislation in India which criminalizes homosexuality. Since ages, the members of LGBTQ community have been discriminated in the society and have been looked upon with shame, hatred and embarrassment by the members of the society and they have gone through what not because of their different sexual orientations and gender identity. In spite of the decriminalization of Section 377 in Navtej Singh Johar’s case in 2018 they are still facing discrimination on daily basis.
