Transient synovitis of the hip remains a common diagnostic problem for the clinician. The physical signs are not pathognomonic of the condition, and the classic technical examinations are of little help. Therefore, the authors retrospectively studied the value of hip arthrosonography in 46 children with clinical symptoms suggesting pathologic hip conditions. In 20 of the 21 patients with a final diagnosis of transient synovitis, articular effusion was detected on ultrasound (US). Conventional radiography showed an increased medial joint space in only eight of these patients. Increased echogenicity of the articular fluid was found in both transient synovitis and septic arthritis. The high sensitivity of US in detecting intraarticular fluid was demonstrated by cadaver studies.
Twenty patients with longstanding polyarticular rheu~ matoid arthritis (including knee involvement) were se~ lected for this study. The severity of the knee synovitis was assessed before and during treatment (with intraarticular corticosteroid injections) using clinical scores, sonography, and thermography. In all patients, the in· flammation regressed during treatment. During followup, the quantity of synovial fluid, as measured sonographically 10 days after the start of therapy, correlated Q uantification of pain and inflammation in rheumatoid arthritis is a difficult clinical problem; 1 as a result, any evaluation based on patient symptoms is fraught with error. MATERIALS AND METHODSSixteen women and four men aged 40 to 73 (mean 59 years) were included in this study. All patients had classical rheumatoid arthritis and were classified according to the grading system of the American Rheumatism As· sociation. u All patients had persistent and disabling knee symptoms despite control of the articular symptoms elsewhere by conventional drug therapy.The mean duration of the rheumatoid arthritis in our group was 10 years (range 2 to 26 years). A syndrome of hyperostosis of the thoracic wall, nonspecific signs of inflammatory disease, and palmar and plantar pustulosis is described in eight patients (Table 1). Seven had intersternocostoclavicular ossification, and one had chronic recurrent multifocal osteomyelitis. This complex of findings has been called "pustulotic arthro-osteitis". This report emphasizes the periosseous soft tissue inflammation and the unexplained subclavian and mediastinal vein thrombosis seen in two patients. Inflammatory periosseous and mediastinal lesions were seen on plain films in all eight patients and on computed tomographic (CT) scans in seven. Radiographs of the spine showed a spondyloarthropathy in three patients. This was characterized by ossification of the vertebral ligaments and sclerosis of the vertebral bodies. Awareness of the radiologic features of pustulotic arthro-osteitis is important because the clinical, biochemical and pathologic findings are often nonspecific and misleading.
