Subject. The article presents the results of the complex treatment of chronic generalized periodontitis associated with fungi of the genus Candida, developed by the authors. The relevance of the study is associated with the prevalence of the disease and the lack of effective treatment. The aim of the study – increasing the efficiency of surgical treatment of patients with chronic generalized periodontitis associated with fungi of the genus Candida, using non-invasive laser blood irradiation in the early postoperative period. Methodology. The study was conducted in two groups of patients (main and comparison group). In the main group at the preoperative stage, a two-component probiotic was included in the complex of anti-inflammatory therapy. Surgical treatment was carried out according to the method developed by the authors, where a plate was used to replace the bone cavity, including collagen and the probiotic Bacillus subtilis 3H immobilized in it. In patients in the comparison group, preoperative anti-inflammatory, as well as surgical treatment, was performed according to a well-known method. The results were evaluated by clinical, immunological and functional research methods. Results. In patients of the main group, a significant improvement in the indicators of clinical, immunological and functional research methods was noted three months after the operation and stably persisted up to 12 months after the operation. In patients of the comparison group, three months after the operation, a negative trend was observed. Findings. Inclusion in the complex of treatment of patients with chronic generalized periodontitis associated with fungi of the genus Candida, a two-component probiotic followed by surgical treatment according to the method developed by the authors using a plate, including collagen and the probiotic Bacillus subtilis 3H immobilized in it, allows achieving stable results in the long-term after surgery. Subject. The antagonistic activity of the Bacillus Subtilis 3H probiotic was studied on a vast amount of data, which makes it possible to rationalize its use in a periodontal practice. The aim of the paper is to substantiate the use of the Bacillus Subtilis 3H probiotic in periodontal practice by studying the antagonistic activity against periodontal-pathogenic microflora and yeast-like fungi of Candida species. Material and methods. The Bacillus Subtilis 3H probiotic sensitivity of reference (archival) strains, clinical isolates from patients with generalized chronic periodontitis associated with yeast-like Candida species fungi, as well as bacteria test cultures and clinical strains of periodontal pathogens, were studied using the deferred antagonism method (diffusion method, perpendicular streak technique). Results. The conducted study made it possible to establish sufficient antagonistic activity of Bacillus Subtilis 3H probiotic against reference strains, clinical isolates isolated from patients with chronic generalized periodontitis associated with yeast-like Candida species fungi, as well as bacteria test cultures and clinical strains of periodontal pathogens. Conclusions. The study findings allow us to recommend the Bacillus Subtilis 3H probiotic to be widely used in the complex treatment of generalized chronic periodontitis caused by periodontopathic microflora, as well as by yeast-like Candida species fungi.
Subject. The imposed restrictions on doctor visits caused by the pandemic Covid-19 and the severe course of the new coronavirus infection have changed the structure of dental care encounters. Objective of the study is to conduct a structural and quantitative analysis of dental care encounters of patients with the previous new coronavirus infection Covid-19. Methodology. A clinical and statistical patient chart was developed for this study. The initial encounters were analyzed in the period from March 2020 to December 2021. We have studied the frequency and structure of dental disease causes in patients who had recovered from the new coronavirus infection Covid-19 depending on the peaks of the Covid-19 incidence. In addition, the age and sex structure, the presence of concomitant diseases, and indicators of the oral cavity state were studied in the main group of patients: the decay-missing-filled index, mouth hygiene index, lesions of the oral mucosa, and regional lymph nodes reaction. The obtained data were processed with the statistical program Statistica 6.0, and differences in subgroups were analyzed according to the Student's t-test: the critical significance level was taken at p ≤ 0.05 when testing statistical hypotheses. Results. This study showed the dependence of the causes, frequency, and structure on pandemic periods. The predominance of the reasons for encounters changed in different periods of the pandemic. Besides, the age structure of patients changed in some periods of the pandemic. At the same time, no dependence of medical encounters on gender composition and comorbidities was observed. Conclusion. This study showed a high level of spread of the carious process. The level of the hygiene index was initially satisfactory, but also moved towards unsatisfactory. Enlargement of lymph nodes and the spread of lesions of the oral mucosa tended to increase during 2020–2021.
