Ovarian vein thrombosis (OVT) is a rare cause of abdominal pain that may mimic a surgical abdomen. It is most often diagnosed during the postpartum period. In this report, we present four cases of postoperative ovarian vein thrombosis. The complications of OVT can be significant, and the diagnosis relies on a careful examination of the radiographic findings. It can occur with lower quadrant abdominal pain, especially in the setting of recent pregnancy, abdominal surgery, pelvic inflammatory disease, or malignancy. Diagnosis can be made with confidence using ultrasound, computed tomography or magnetic resonance imaging. Treatment of ovarian vein thrombosis is particularly important in the post-partum patients, with anticoagulation therapy being the current recommendation.
Desmoid tumors are a rare group of locally aggressive, non malignant tumors of fibroblastic origin that can lead to significant morbidity due to local invasion and may even result in a fatal outcome when located around vital organs. Their clinical presentation, biological behavior and natural history can be quite varied and is incompletely understood at the present time. The optimal therapeutic approach depends on various factors, and a multidisciplinary approach is necessary to achieve local control with acceptable morbidity. Despite progress in the understanding of these tumors and the treatment options, local recurrence remains a major problem.
Nous rapportons dans ce travail, le profil épidémiologique des femmes enceintes porteuses d'une cardiopathie ayant accouché dans le service «A» du centre de maternité de Tunis. Il s'agit d'une étude rétrospective descriptive sur 3 ans, de janvier 2010 à décembre 2012, portant sur les dossiers des parturientes cardiaques suivies et ayant accouché dans notre service, à l'exclusion des cardiopathies hypertensives. Les paramètres analysés étaient la gestité et parité des parturientes, le type de la cardiopathie, le mode d'accouchement et le pronostic foetal. Cinquante six cas sur 19655 accouchements avaient été recrutés, soit une incidence de 1 pour 351 accouchements (0,285%). L’âge moyen de nos parturientes était de 30,89±5,3 ans avec des extrêmes allant de 21 à 42 ans. 23 (41,07%) avaient une cardiopathie congénitale, 35 (62,5%) une valvulopathie, 6 (10,71%) un trouble du rythme, 3 (5,35%) un trouble de la conduction à type de bloc auriculo-ventriculaire et 3 (5,35%) une cardiopathie ischémique. Cinquante deux parturientes (92,85%) avaient accouché à un terme dépassant les 37 semaines d'aménorrhée (SA). L'accouchement s'est déroulé par les voies naturelles chez 22 (39,28%) patientes et par césarienne chez 34 (60,71%). Le poids moyen des nouveaux nés à la naissance était de 3341,25 grammes. 3 transferts néonataux en unité de réanimation étaient réalisés avec un seul cas de décès néonatal. La grossesse et l'accouchement chez la femme cardiaque constituent un haut risque materno-foetal dépendant du type, mais surtout du stade de la cardiopathie, nécessitant une prise en charge précoce et multidisciplinaire de la parturiente.
Uterine leiomyomas are very common tumors found in women. Rupture of veins on the surface of uterine leiomyoma is an unusual source of hemoperitoneum. It is an extremely uncommon gynaecological cause of hemoperitoneum. It is a life threatening emergency. We report a case of massive intraperitoneal hemorrhage due to rupture of vessels on the surface of subserous leiomyoma. A differential diagnosis of rupture of leiomyoma’ssurface vessel should be considered, while dealing with a case of hemoperitoneum with pelvic mass.
Ovarian pregnancy (OP) is rare: it accounts for 3.2% of ectopic pregnancies. We here report the case of a 36-year old patient gravida 3, para 2. The only risk factor for ectopic pregnancy found in this patient was a history of intrauterine contraceptive device (IUCD) use. Clinical symptoms were dominated by the occurrence of pelvic pain at 5-weeks of amenorrhoea. At the initial visit to the Emergency Department ß-Human chorionic gonadotropin (HCG) was 2134 IU/L. Preoperative diagnosis of ovarian pregnancy was suspected due to a rounded image measuring 26 mm with double echogenic crown visible on the left ovary without associated free fluid in the pouch of Douglas on ultrasound. Treatment was based on coelioscopy with resection of the ectopic chorionic villi in the left ovary. Medical treatment was based on intramuscular injection of methotrexate 1 mg/kg body weight in the immediate post-operative period because of the risk of a little residual of trophoblastic material. The diagnosis was confirmed by the histological study of the surgical specimen. The postoperative course was uneventful
