BackgroundPrimary dysmenorrhea is a common and sometimes disabling condition. In recent years, some studies aimed to improve the treatment of dysmenorrhea, and therefore, introduced several therapeutic measures. This study was designed to compare the analgesic effect of iron chip containing heat wrap with ibuprofen for the treatment of primary dysmenorrhea.MethodsIn this randomized (IRCT201107187038N2) controlled trial, 147 students (18–30 years old) with the diagnosis of primary dysmenorrhea were enrolled considering the CONSORT guideline. Screening for primary dysmenorrhea was done by a two-question screening tool. The participants were randomly assigned into one of the intervention groups (heat Patch and ibuprofen). Data regarding the severity and emotional impact of the pain were recorded by a shortened version of McGill Pain Questionnaire (SF-MPQ). Student's t test was used for statistical analysis.ResultsThe maximum and minimum pain severities were observed at 2 and 24 hours in both groups. The severity of sensual pain at 8, 12, and 24 hours was non-significantly less in the heat Patch group. There was also no significant difference between the groups regarding the emotional impact of pain at the first 2, 4, 8, 12 and 12 hours of menstruation.ConclusionsHeat patch containing Iron chip has comparable analgesic effects to ibuprofen and can possibly be used for primary dysmenorrhea.Trial registrationIRCT201107187038N2
Background:Qualitative research methods can help investigators ascertain the depth of people's needs and their perceptions. This study was designed to describe mothers' experiences and perceptions of lay doula services during labor and delivery.Materials and Methods:This study was conducted using a qualitative approach and conventional procedures of content analysis. The participants consisted of 13 nulliparous women at three hospitals affiliated with Zahedan University of Medical Sciences in 2016. Data were collected using face-to-face semi-structured interviews. Purposive sampling continued until data saturation was ensured. All interviews were tape-recorded and transcribed in verbatim.Results:Participants' experiences were categorized into 11 subthemes and five major themes including (1) achieving self-esteem and empowerment, (2) more trust in God, (3) promoting mental health of the mother, (4) willingness toward normal childbirth, and (5) lay doula as a listener and perceiver.Conclusions:In our study, the mothers evaluated the presence of a lay companion as an effective helper during childbirth and someone who promoted a remarkable willing toward normal childbirth. Healthcare professionals must be cognizant of the needs, values, beliefs, preferences, and emotional well-being of disadvantaged women during labor and delivery in poverty-stricken areas of Iran. Also, this study highlighted that it is important to include the perspective of support persons (such as husbands) in research design of future studies.

Objective: To determine the incidence, and some causes of intra uterine fetal death (IUFD).
Methods:This was a prospective observational study of all stillbirths over 2 years from April 2011 to April 2013 at the Imam Ali University Teaching Hospital of zahedan, Iran. The variables studied were the following: demographic characteristic, fetal factors and maternal factors. Data was collected prospectively by directly interviewing the pregnant women and from the medical chart. Data were analyzed using SPSS (Statistical Package for the Social Sciences) statistical package.
Results:The incidence of IUFD at our hospital was 88.7 per 1000 total births. The intra uterine fetal death rate had been increased in maternal ages under 20 years and above 35 years that showed a significant difference between this group and other groups (P=0.001).The rate of IUFD tend to increase significantly in primiparous women and women with parity 10 and more. The rate of IUFD tended to decreased with increasing gestational age. There were 437 (54.1%) macerated and 370 (45.8%) fresh death fetus. Unexplained' IUFD, Major congenital malformations, PROM, and preeclampsia were the three main causes of intra uterine fetal death.Discussion: Unfortunately, in our study the incidence of IUFD was very high. The main risk factors identified were lack of antenatal care. Despite the difficulty in predicting IUFD occurrence, it appears that carefully implemented antenatal care, family planning, genetic counseling and timely management of at risk patients may contribute to its prevention.
