The Brazilian Ministry of Health launched its Family Health Program (FHP) in 1994 as a new strategy aimed at reorganizing the healthcare system starting from the primary care level. The Program prioritizes delivery of care to groups identified as presenting increased risk, such as diabetic and hypertensive individuals. The objective of this study was to evaluate the care delivered to such patients by professionals in this program in the municipality of Francisco Morato, São Paulo, Brazil. A random cluster-based sample selected 84 patients with diabetes and/or hypertension who answered a questionnaire at home. Seventy-two patients responded. Of these, 19% were diabetic, 41.7% hypertensive, and 38.9% had both diabetes and hypertension. As for the care offered by the FHP, only 35.9% received excellent diagnostic workups and 10.9% excellent physical examinations. Some 20.8% of the patients reported illness-related complications. With the implementation of the FHP, access to care improved for both diabetic and hypertensive patients. The FHP proved to be a valid alternative to increase healthcare access, seeking to achieve greater equity by delivering care in accordance with the population's needs.
Resumo: Objetivou-se avaliar a implementação da Linha de Cuidado (LC) em hipertensão arterial e diabetes mellitus em uma Região de Saúde do estado de São Paulo. Trata-se de uma pesquisa avaliativa desenvolvida em três etapas: caracterização das regiões para seleção da região-caso; entrevistas com gestores e profissionais de saúde que atuam na Atenção Básica, Média e Alta Complexidade e entrevistas com pacientes em Ambulatórios Médicos de Especialidades. A análise baseou-se em um modelo lógico que considera a influência da macro e micropolítica na construção da LC. De modo geral, insuficiências foram identificadas nas dimensões da macropolítica (organização da rede de serviços e utilização de protocolos) e micropolítica (coordenação do cuidado pela AB, vinculação e responsabilização do cuidador). Conclui-se que, apesar dos avanços alcançados, grandes desafios se impõem para o alcance da integralidade da atenção à hipertensão arterial e diabetes mellitus, sendo necessários investimentos nos processos da macro e micropolítica. Arterial Hypertension (AH) and Diabetes Mellitus (DM) are considered a worldwide epidemics whose control poses a challenge to health care services. Within the National Health System, the Family Health Program currently has the dual role of being a system gateway and reference structure. Bearing in mind this scenario, this study assessed the access of AH and DM patients to health care and therapeutic drugs. A household survey was conducted in five municipalities with over 100,000 inhabitants in the Baixada Santista. A two-stage self-weighted probabilistic sample was used. Results estimated a prevalence of 26.3% for AH and of 8.8% for DM, AH being more prevalent among women. As to health care, 85.3% of the individuals with AH interviewed reported having had their arterial pressure checked, and 70.2% of those with DM reported having had their glucose blood serum level tested in the preceding six months. Drug treatment was prescribed to 99.4% of these patients, and 62.8% of AH patients purchased such drugs from private drugstores, and 57.9% of DM patients received drugs provided by health centers. Over 90% of the patients had no access to educational group activities, and 78% of AH and 92.5% of DM patients had no supervising home visits. These findings suggest the need for primary health care as the mainstay for the care provided to HA and DM.
Objective The purpose of this study was to evaluate the perinatal outcomes in pregnant women who use illicit drugs. Methods A retrospective observational study of patients who, at the time of delivery, were sent to or who spontaneously sought a public maternity hospital in the eastern area of São Paulo city. We compared the perinatal outcomes of two distinct groups of pregnant women - illicit drugs users and non-users - that gave birth in the same period and analyzed the obstetric and neonatal variables. We used Student's t-test to calculate the averages among the groups, and the Chi-square test or Fisher's exact test to compare categorical data from each group. Results We analyzed 166 women (83 users and 83 non-users) in both groups with a mean of age of 26 years. Ninety-five percent of the drug users would use crack or pure cocaine alone or associated with other psychoactive substances during pregnancy. Approximately half of the users group made no prenatal visit, compared with 2.4% in the non-users group (p < 0.001). Low birth weight (2,620 g versus 3,333 g on average, p < 0.001) and maternal syphilis (15.7% versus 0%, p < 0.001) were associated with the use of these illicit drugs. Conclusions The use of illicit drugs, mainly crack cocaine, represents an important perinatal risk. Any medical intervention in this population should combine adherence to prenatal care with strategies for reducing maternal exposure to illicit drugs.
