BACKGROUND Many rheumatologic abnormalities have been described in patients with classical symptomatic primary hyperparathyroidism, but none of these are commonly seen. They include the pseudogout, with pyrophosphate crystals into the joint and calcification of articular cartilage (chondrocalcinosis), most commonly affecting the wrists and knees.
A colecistite acalculosa é uma inflamação da parede da vesicular biliar sem a presença de calculos, sendo responsável por 4 a 10% por cento dos casos de colecistite aguda. Objetivo: relatar um caso de CAA no serviço do HPP. Métodos: estudo restrospectivo de relato de caso. Discussão: Paciente de 13 anos, masculino, previamente hígido, admitido no HPP com quadro de dor em flanco direito, associada a náuseas, inapetência e diarréia líquida há 1 semana, com melhora parcial ao uso de analgésicos comuns (paracetamol e dipirona), porém com recorrência da dor. Realizou ultrassonografia de abdome total que mostrou colecistite aguda alitiásica e exames laboratorias com sinais inflamatórios. Paciente foi internado para tratamento conservador com antibioticoterapia. Apresentou resolução do quadro após 1 semana de tratamento, tendo alta hospitalar e sendo encaminhado para investigação ambulatorial. Conclusão: A CAA é uma patologia rara, de baixa prevalência na população geral e principalmente pediátrica. Deve-se suspeitar especialmente em pacientes críticos e hospitalizados por longos períodos, contudo, casos em pacientes previamente hígidos podem ser não tão infrequentes.
BACKGROUND Clinical syndromes may be associated with deposition of calcium pyrophosphate crystals dihydrate (CPP) in connective tissues. Crowned dens syndrome (CDS) is a rare presentation involving the presence of CPP around the atlantoaxial joint, which is characterized by acute or recurrent severe cervical pain, neck rigidity, and associated fever, in addition to increased inflammatory markers. Identifying CPPs by computed tomography (CT) is essential to exclude the differential diagnosis, such as polymyalgia rheumatica, giant cell arteritis or, less frequently, meningitis, cervical dyscitis or inflammatory spondyloarthritis We present a case report of a male with recurrent cervical pain irradiated to the shoulder and upper limbs, with reduced cervical mobility, whose MRI indicated CDS.
