Micro-invasive 360-degree suture trabeculotomy represents a simple and safe minimal invasive ab interno technique to postoperatively enhance the IOP-lowering and drug-sparing effect of canaloplasty.
During vacuum application of the liquid patient interface of the Femto LDV Z8 for femtosecond laser-assisted cataract surgery, IOP values were higher in the anterior chamber compared with the intravitreal pressure measurements. The higher predefined vacuum level (350 mbar vs 420 mbar) resulted in significant higher intracameral IOP.
The modified surgery technique of canaloplasty with suprachoroidal drainage yields the opportunity to optimize the IOP-lowering effect of canaloplasty as a non-penetrating surgical procedure while still offering a more favourable safety profile than trabeculectomy. The suprachoroidal access facilitates the most complicated step of the canaloplasty surgery: the localization of Schlemm's canal.
