Acutely psychotic schizophrenics were randomly assigned to one of three groups: an experimental group of patients home on drugs and public health nursing care, and two control groups—one receiving placebo and public health nursing home care, the other receiving state hospital care. For a thirty‐month period it was demonstrated that over three‐quarters of the experimental group could be successfully maintained at home. Five years later, gradual erosion of the original significant differences occurred on the usual clinic and aftercare services, so that eventually no differences in social or psychological functioning could be found. This indicates a need for the structuring of community mental health services on an intensive aggressive basis, if we are to anything more than transfer custodial care to the community.
