Since the installation of a computed tomography (CT) scanner at a local organ procurement facility, CTs have been performed on potential donors who suffered terminal events and used, in conjunction with other evaluative protocols, to direct donation decisions and assist in procurement procedures.A retrospective analysis of data from 373 cases spanning five years was systematically collected from the procurement facility's online database and analyzed. This data included patient medical histories, biopsy results, operative findings, and CT results for liver transplants.CT findings resulted in the rejection of 25% of the discarded livers. Identification of liver steatosis and cirrhosis by CT was compared to prior mention in a patient's medical history, and CTs identified more patients with findings concerning for underlying pathology. Every liver found by CT evaluation to be cirrhotic was rejected for transplant, 75% of which did not undergo subsequent biopsy. Of the CTs obtained with contrast, CT findings of variant hepatic arterial anatomy agreed with operative findings in 75.8% of cases (p < 0.0001). CT-determined liver volumes were found to be statistically accurate when compared to operative findings (p < 0.0001).This study suggests that using CT scans may help facilitate the organ directive decision making process by helping match donors and recipients, and maximize efficiencies in the operating room by reducing the cost and time spent on each case.
