BACKGROUND Autoimmune hepatitis (AIH) is an immune-mediated liver disease affecting all age groups. Associations between hepatitis A virus (HAV) and AIH have been described for many years. Herein, we report a case of an AIH/primary biliary cholangitis (PBC) overlap syndrome with anti-HAV immunoglobulin M (IgM) false positivity. CASE SUMMARY A 55-year-old man was admitted with manifestations of anorexia and jaundice along with weakness. He had marked transaminitis and hyperbilirubinemia. Viral serology was positive for HAV IgM and negative for others. Autoantibody screening was positive for anti-mitochondria antibody but negative for others. Abdominal ultrasound imaging was normal. He was diagnosed with acute hepatitis A. After symptomatic treatment, liver function tests gradually recovered. Several months later, his anti-HAV IgM positivity persisted and transaminase and bilirubin levels were also more than 10 times above of the upper limit of normal. Liver histology was prominent, and HAV RNA was negative. Therefore, AIH/primary biliary cholangitis (PBC) overlap syndrome diagnosis was made based on the “Paris Criteria”. The patient was successfully treated by immunosuppression. CONCLUSION This case highlights that autoimmune diseases or chronic or acute infections, may cause a false-positive anti-HAV IgM result because of cross-reacting antibodies. Therefore, the detection of IgM should not be the only method for the diagnosis of acute HAV infection. HAV nucleic acid amplification tests should be employed to confirm the diagnosis.
The indigenous tribal peoples have been living in the forests for millions of years and their livelihood depends on the forest resources, they have symbiotic relationship with nature, before independence there is no much restriction to tribal's in the forest and they can live freely and use the forest resources. Government brought out different forest policies and declares the most of the forest areas into Wildlife Sanctuaries, National Parks, Tiger Reserves, Community Reserves and reserve forest to protect and conserve forest, it helps the wildlife and forest conservation and in another hand it has impacted the livelihood of the millions of tribal's. The major livelihood occupation of Soliga tribal's are Non-Timber Forest Products collection and sale and they also engaged in agriculture, labour and livestock rearing, they are earn income from different sources that 43 percent from labour, 45 percent from agriculture, 7 percent from sale of forest resources and 5 percent from sale of livestock. BRT reserve forest declares into wildlife sanctuary in 1974 and stopped the shifting cultivation and displaced settlements to periphery and allow the some of the settlement to stay inside the forest, NTFPs collection was banned in 2006 and Government did not provide any alternative employment and due to this 32.5 percent of them migrated outside for employment, it made adverse affect on Soliga tribal economy. The recent policy of Forest rights Act 2006 given some space for tribal's to get rights to collect NTFPs and use the forest land, even that also they are facing the relocation problem because of declare into Tiger Reserve in 2011. The study will focus on impact of forest policies and economic scenario of the Soliga tribes.
Institutional care for adolescent girls can be regarded as a form of care is provided by paid caretakers in group care. It is a temporarily organized residential care for children and adolescents. An institution is a place where alternative care is provided who are in need of care, support and protection. Institutional care for adolescent girls at young age disrupts the normal development. Adolescence stage is referred as a period of stress and storm. It is a time for the occurrence of physical and sexual changes and these changes may cause psychological discomfort among adolescents. Under such circumstances getting adjusted to the institutional care in the absence of parental supervision is a very difficult one. No human being can adopt himself to his own physical environment without proper adjustment. Adjustment is a dynamic and continuous process. It demands the abilities of adolescents to face various challenges in everyday life and if they are able to cope up with the challenges, they will be well adjusted otherwise it may lead to mal adjustment. The present study aimed to determine the different dimensions of adjustment such as home, peer, school, teacher and general adjustment among adolescent girls living in institutions. The result revealed that many of them have adjustment problem while residing in the institution.
