A 64-year-old man was admitted to the emergency center of Furukawa City Hospital because of common cold-like symptoms and hypotension. He was diagnosed as fulminant myocarditis with cardiogenic shock and arrhythmia elicited by influenza virus subtype A. Cardiac angiography, echocardiography and biopsy also showed myocarditis, and serum antibody titer to influenza virus subtype A was increased to 4-fold in paired serums. Treatments of both percutaneous cardio-pulmonary support (PCPS) and intra-aortic balloon pumping (IABP) were carried out to sustain the general circulation. PCPS treatment was discontinued on the 25th day of the admission, but IABP was continued. Finally, he died of multiple organ failure. The autopsy revealed myocardial necrosis with a slight fibrosis and a small amount of lymphocytic infiltration into the ventricular wall, which were compatible with restrictive myocarditis. Moreover, immunohistochemical analysis also showed the presence of viral antigens in cardiac myocytes. This case clearly showed that PCPS and IABP can be beneficial to sustain the general circulation in fulminant myocarditis, but cardiac pumping function failed completely to recover from myocardial damage.
Natriuretic Polypeptide (ANP) as Protective Agent of Renal Ischemia. Tohoku J. Exp. Med., 1989, 158 (4), [283][284][285][286][287][288][289] The effect of atrial natriuretic polypeptide (ANP) on hemodynamics and renal function was evaluated after the reconstructive surgery of the left renal artery in a patient with renovasular hypertension secondary to Takayasu's arteritis. The reconstructive surgery was done using the femoral artery, since we were unable to obtain adequate vein segments to fit the renal artery. The femoral artery was reconstructed by her saphenous vein segments. After 30 min of the aortorenal bypass operation, a-human ANP (ahANP) was infused intravenously for 10 min at a rate of 0.1pg/kg/min. Although total peripheral resistance was decreased by a-hANP infusion, blood pressure was not changed because of the increased cardiac output. Glomerular filtration rate was increased markedly with concomitant increases in urine volume and urinary excretions of sodium, potassium and phosphate. Fractional excretions of water and sodium were not changed, but fractional excretion of phosphate and potassium clearance were increased. Thus, the infusion of a-hANP markedly improved the renal function of the ischemic kidney by the reconstructive surgery of the renal artery, suggesting that a-hANP seems clinically applicable as a protective agent in renal ischemia at renovascular surgery as well as the renal transplantation.atrial natriuretic polypeptide (ANP) ; renal ischemia ; renovascular hypertension ; reconstructive surgery It is extremely important to avoid the ischemic renal damage for the preservation of renal function at the reconstructive surgery of the renal artery. Synopsis : Fe-36Ni(Invar) steel sheet is used for shadow masks in color cathode ray tubes. For using shadow masks, it is necessary to obtain a low yield strength, high r-value and excellint etching property after annealing. So, nuclei orientations of recrystallization, and grain growth of invar alloy were studied by transmission electron microscopy (TEM). The results are as follows :( 1 )In case of cold rolling with light cold rolling reduction and annealing, nuclei orientations of recrystallization are mainly ( 011) and ( 011) nuclei orientations have matrices of (011) orientation.( 2 )In case of cold rolling with heavy cold rolling reduction and annealing, nuclei orientations of recrystallization are mainly ( 011) and ( 011). Both orientations have matrices of (011) orientation.( 3 )The grains of (011) orientation in sample with heavy cold rolling reduction are increased with progressing annealing.
