IntroductionFibromyalgia is difficult to treat and requires the use of multiple approaches. This study is a randomized controlled trial of qigong compared with a wait-list control group in fibromyalgia.MethodsOne hundred participants were randomly assigned to immediate or delayed practice groups, with the delayed group receiving training at the end of the control period. Qigong training (level 1 Chaoyi Fanhuan Qigong, CFQ), given over three half-days, was followed by weekly review/practice sessions for eight weeks; participants were also asked to practice at home for 45 to 60 minutes per day for this interval. Outcomes were pain, impact, sleep, physical function and mental function, and these were recorded at baseline, eight weeks, four months and six months. Immediate and delayed practice groups were analyzed individually compared to the control group, and as a combination group.ResultsIn both the immediate and delayed treatment groups, CFQ demonstrated significant improvements in pain, impact, sleep, physical function and mental function when compared to the wait-list/usual care control group at eight weeks, with benefits extending beyond this time. Analysis of combined data indicated significant changes for all measures at all times for six months, with only one exception. Post-hoc analysis based on self-reported practice times indicated greater benefit with the per protocol group compared to minimal practice.ConclusionsThis study demonstrates that CFQ, a particular form of qigong, provides long-term benefits in several core domains in fibromyalgia. CFQ may be a useful adjuvant self-care treatment for fibromyalgia.Trial registrationclinicaltrials.gov NCT00938834.
This extension trial is an open-label observational trial of 20 subjects with fibromyalgia who undertook level 2 Chaoyi Fanhuan Qigong (CFQ) training following an earlier controlled trial of level 1 CFQ. Subjects practiced 60 min/day for 8 weeks and continued some daily practice for 6 months. Quantitative measures, assessed at baseline, 8 weeks, 4 and 6 months, were of pain, impact, sleep, physical and mental functions, and practice time. Qualitative comments also were recorded. Compared to baselines, CFQ practice led to significant improvements in pain, impact, sleep, and physical function in the 13 subjects (65%) who completed the trial; changes were present at 8 weeks and were maintained for the 6-month trial duration. A highly motivated subgroup of N = 5, who practiced the most, had the best outcomes in terms of end symptomology, and qualitative comments indicated health benefits in other domains as well. Qualitative comments by the remaining N = 8 trial completers and N = 7 withdrawals indicate different experiences with the practice. This extension trial indicates that diligent CFQ practice over time produces significant health gains in fibromyalgia in a subset of individuals. Future studies will need to address factors that might predispose to favourable outcomes.
The case study approach provides data with respect to extent of practice, perseverance and long-term outcomes, and provides valuable insight into the potential of this self-care practice.
Background: Qigong, traditional Chinese mind-body practices, is currently receiving increasing attention with exploration of what it is and what it does. Common elements include postures and movement, attention to breathing, and mental instructions. There is a growing literature on the health benefits of qigong in chronic pain and other conditions, and an intervention that provides benefit in multiple domains is particularly important for clinical practice. In this report, we present a case series of six individuals who engaged in extended qigong practice over the course of at least 4 and up to 11 years, with data derived from an observational trial which evaluated effects of qigong practice on pain, mood, sleep, fatigue, quality of life and overall health in patients attending an outpatient pain management clinic. Methods: Participants attended weekly qigong classes as a voluntary, complementary practice for chronic pain management, and were encouraged to practice regularly at home. Qualitative data were collected prospectively over a series of sessions. Participant narratives were reviewed and those with extended practice histories were included in our case series. Results: Qualitative commentary reflected the richness and heterogeneity of benefits achieved in all studied health domains. Three primary themes were identified: 1) Direct benefits of qigong practice include improvements in pain, pain control, sleep, and other health areas; 2) Regular qigong practice improves quality of life, increases energy and activity, improves mood, and decreases use of medications; 3) Qigong practice cultivates an improved outlook on life and the future as an overall effect. Conclusions: This case series reveals multiple health benefits of extended qigong practice. Given that these occurred in those who practiced the most, one of the barriers to potential health benefits achieved by qigong is commitment to the practice. In appropriate circumstances, health care professionals can recommend qigong as a complementary practice for successful chronic pain management.
Qigong, which can be characterized in many different ways, is offered as a complementary self-care practice at a tertiary-care pain management unit in Halifax, Nova Scotia. This report provides a quantitative and qualitative assessment of two groups engaged in qigong practice in this context as part of two Research in Medicine (RIM) projects (2014-15, 2016-17). It includes assessments of pain, mood, quality of life, sleep, and fatigue, considers outcomes in relation to the amount of practice, and considers whether health attitudes would help determine who might benefit from the practice. There were 43 participants (28 ongoing practitioners, 15 new to qigong). The ongoing practice group in RIM2 had significant benefits over time in pain, mood, quality of life, and fatigue in quantitative scores, but changes were not significant in RIM 1. There were no differences in any measures in those new to qigong. Qualitative comments in core and other domains reflected good or better outcomes in 16 subjects in the ongoing group who practiced consistently. In those who practiced less, results were more variable. In most of those new to qigong, the practice was limited and comments indicate minimal changes. Those engaged in qigong have a stronger internal health locus of control than control subjects. Diligent qigong practice provides multiple health benefits for those with chronic pain, and qualitative assessments are essential for documenting these effects. For those new to qigong, factors needed to effectively engage practice need to be explored further to optimize program delivery.
