Successful implementation of the state program «Health Development» requires a high level of competence in the strategic development of medical organizations and the healthcare industry from managerial personnel. In the current practice in the healthcare system, a person with experience in medical work is often appointed to the position of head, and he has to acquire managerial knowledge and skills directly in the process of activity. The goal of training managerial personnel in healthcare is to master uniform standard professional competencies, which should become the basis of educational programs for additional professional education in all educational organizations. Target. Analysis of the educational activities of the GAUDPO “Institute of Health Management named after A.I. A.B. Blokhin» for the implementation of additional professional education in the field of healthcare. Materials and methods. The study used regulatory legal documents that regulate the issues of training and continuous development of managerial personnel in the healthcare sector, methods of systemic and logical analysis. Analyzed, grouped and statistically processed the results of a survey of students of additional professional education programs in the specialty «Health Organization and Public Health» (255 questionnaires). Results. The actual practice of the activities of medical organizations requires the improvement of approaches to the training of healthcare organizers. The Institute is actively working to develop relevant programs for additional professional education and filling the labor market of the Sverdlovsk region with professional personnel in the field of healthcare. Conclusion. An analysis of the implementation of additional professional education programs by the Institute showed that against the background of mandatory professional retraining programs of 500 hours or more, short-term programs are gaining popularity, allowing them to acquire and “hone” relevant knowledge, skills and abilities. All the main trends of modern additional professional education are implemented by the Institute, taking into account the requests of the main participants in the educational process and the requirements of the health management body — the Ministry of Health of the Sverdlovsk Region.
Regional centers for the organization of primary health care play an important role in the implementation of measures to create a New model for organizing the provision of medical care at the subject level. The purpose of the work: to analyze the experience of organizing the work of the Regional Center for the Organization of Primary Health Care in the Sverdlovsk Region for the period 2018–2021. Materials and methods. The work uses systematic and integrated approaches, analytical, statistical methods. Results. Based on the results of the assessment, it was found that the Regional Center for the Organization of Primary Health Care forms a general strategy for the implementation of lean technologies in healthcare in the region, coordinates, and involves the heads and staff of medical organizations in the processes of lean management of a medical organization, including through training in lean production methods. Based on the analysis of activities, an algorithm for organizing the work of the Regional Center for the Organization of Primary Health Care is presented. Also, a high level of implementation of the New Model of Organization of Medical Care for 5 years was determined. Conclusion. The creation of the Regional Center for the Organization of Primary Health Care on the basis of an organization carrying out educational and scientific activities provides great opportunities for training employees of medical organizations, and allows the successful implementation of the New Model of Organization of Medical Care.
Introduction. Tobacco smoking is the most dangerous and significant risk factor for diseases of the circulatory system and an increase in premature mortalityAim. To study the gender aspects of the prevalence of smoking in connection with the sex and age characteristics and the level of education of patients with diseases of the circulatory system.Materials and methods. A total of 422 people were interviewed (267 women and 155 men). Sample – target spontaneous filling; the questionnaire was filled out in paper form at an outpatient appointment.Results. Among the surveyed patients with diseases of the circulatory system, the fact of smoking was confirmed by 29% of men and 6.4% of women. The most common image of a smoking person is a man aged 30-49 years (in this group, 55.6% smoke). Smoking is widespread among men aged 50 and older (every fourth smokes). Among women, the most susceptible to smoking is the age group of 30-49 years (15.3% smokers). Typical quitters include men aged 50-69 (58%) and women aged 30-49 (15.3%). The absence of the habit of smoking tobacco is most characteristic of the generation under the age of 29 years. According to the educational level, men with incomplete secondary and secondary education are susceptible to smoking (47.4% and 34.9%, respectively). Among men with secondary, secondary specialized and higher education, the share of those who quit smoking was relatively high (51.2%, 59.3% and 50%, respectively). The prevalence of tobacco smoking among women compared to men by educational groups is significantly lower: among people with secondary education – by 5.7 times (6.1% vs. 34.9%), with secondary specialized education – by 2.4 times (7.8% vs. 18.6%), with higher education – 5.2 times (5.6% vs. 29.4%).Conclusion. The data obtained update the development of programs for the prevention of smoking among patients with diseases of the circulatory system, taking into account gender, age and educational groups.
Nowadays, creation of a new medical organization model providing primary healthcare services (New Model) is broadly associated with the introduction of lean manufacturing technologies. In the setting of modern healthcare development, the challenge of evaluating the efficiency of the medical organization activities remains primary and unresolved, especially in the context of introduction of new methodological approaches. The article describes the experience of creating and evaluating the efficiency of the New Model implementation in medical organizations of the Sverdlovsk region operating in the compulsory medical insurance system.The purpose of the study. To present the results of evaluating the effectiveness of the implementation of the New Model in medical organizations in the Sverdlovsk region.Research materials. The activities of 79 medical organizations involved in the New Model creation and expansion in the period between 2017 and 2019 were analyzed.Research results. The rate of staff involvement in the medical organizations was examined using an express “Questionnaire Q12” method; herewith 1,489 employees were interviewed in 2018, and 3,640 employees in 2019. The level of patient satisfaction with the medical care quality and availability, the number and amounts of fines/deductions/withdrawals for the outpatient care services collected by insurance medical organizations (IMOs), Territorial Fund of Compulsory Medical Insurance of the Sverdlovsk Region (TFCMI SR) were studied.Conclusion. The New Model implementation increases the level of patient satisfaction with the medical care quality and availability, the number of fines/deductions for the outpatient care services collected by IMOs, TFCMI SR has been decreased. A high staff involvement in the medical organization activities has been observed, however certain activities aimed at satisfying the HR needs and staff motivation need to be developed. It is necessary to continue further evaluation of the efficiency of the New Model implementation in terms of medical, social and economic indicators. Crucial in the New Model implementation is the interaction between TFCMI SR, IMOs, and medical organizations in the provision of primary healthcare services aimed at the assessment of the medical care availability and quality, including patient satisfaction.
Introduction: Consideration of the problems of prevention, diagnosis and treatment of patients is associated with the study of the subjective opinion of medical workers and the assessment of their role in the development of population health care. The objective of the study was to study the opinion of medical workers of the Sverdlovsk Region on the availability and quality of care for patients with diseases of the circulatory system. Methods: In February – March 2020, we conducted a questionnaire-based survey of 303 randomly chosen health professionals, mainly general practitioners and cardiologists, working in urban and rural health care facilities of the region. Results and discussion: The majority of the respondents believed that the patient’s adherence to prescribed treatment and rejection of bad habits contributed the most to mortality reduction. Standards of medical care and Russian clinical guidelines were reported as the most significant factors in making decisions on patient management. The respondents rated their own competence level quite low in determining the need for medical interventions. When assessing the quality of the follow-up care, the respondents reported regular monitoring of only four parameters (electrocardiogram, platelet and lipoprotein levels, and body weight), explaining the incompleteness of the process by limited opportunities for a proper examination in medical facilities. The health care providers noted the observance of sequencing (routing) in provision of medical care and the possibility of obtaining the necessary medicines; yet, almost half of the respondents specified that the recommended clinical examinations at the outpatient stage of treatment were conducted only in part. When assessing the effectiveness of work of healthcare centers for patients with chronic heart failure, positive changes were noted by 25 % of the respondents while 20 % reported a reduction in waiting time for high-tech care. Conclusion: The study of the opinion of medical workers helps reveal contradictions in rendering health services. Possessing a well-formed professional mindset, they reflect official (insider’s) views on the problems of the healthcare system in their assessments, but being involved in communication with patients, they express philistine opinions.
