Objective:To identify opportunistic screening actions for breast cancer performed by nurses working in primary health care units in Ribeirão Preto, São Paulo. Method: Cross-sectional study with 60 nurses from 28 units, who had been working for at least one year in the public municipal health care network. Data were collected between December 2013 and March 2014, by means of a questionnaire, using descriptive analysis and the software IBM SPSS version 20 and Microsoft Excel 2010. Results: The results showed that 71.7% of the participants questioned their female patients as for risk factors for breast cancer, mainly during nursing consultation; 70.0% oriented users about the age to perform clinical breast exam, whereas 30.0% did not due to lack of knowledge and time; 60.0% explained about the age to perform mammogram; 73.3% did not refer patients with suspicious breast exam results to the referral department, citing scheduling as the main obstacle to referral. Educational activities were not performed by 78.3% of participants. Conclusion: Investment is needed in professional training and management of breast cancer screening. OBJECTIVE: To evaluate the attributes of Primary Health Care (PHC) for rural workers; to analyze sociodemographic conditions, history of poisoning and hospitalizations for pesticides and use of personal protective equipment; and to verify exposure to pesticides by determining bioindicators. METHODS: Cross-sectional, descriptive-analytical study with a sample of 1,027 rural workers living in municipalities belonging to a regional health department in Southern Minas Gerais, whose PHC is governed by the Family Health Strategy model. We used the adult version of the Primary Care Assessment Tool (PCATool Brazil) and a structured questionnaire to collect socioeconomic data, history of poisoning and hospitalization for pesticides and use of personal protective equipment. Blood samples were collected to measure biomarkers of pesticide exposure and signs of renal and hepatic sequelae. RESULTS: Low education was prevalent, as well as the intense contact of workers with pesticides. Frequent use of personal protective equipment was higher among men, as was the history of poisoning and hospitalizations for pesticides. Rates of 20% poisoning, 15% liver disease and 2% nephropathy were detected. Signs of hepatotoxicity were more frequent in men. Gender differences were all statistically significant. Regarding PHC, only the attribute “degree of affiliation” had a high score. None of the poisoning cases detected in the study were previously diagnosed. CONCLUSIONS: Despite the high coverage of the Family Health Strategy, occupational risk and its consequences have not been detected by health services, which do not seem oriented to primary care, even lacking their essential attributes. There is a need for immediate and effective adaptation of public policies regarding the health of rural workers, with adequate training of teams and review of the portfolio of PHC services offered.
Objective: To understand the experience of partners of women with breast cancer. Method: This was a qualitative study, based on the theoretical-methodological framework of existential and phenomenological psychology, and carried out with the partners of women with breast cancer who attended a center for rehabilitation after mastectomy. The data were collected in interviews between June and November 2014, based on a guiding question. Results: Ten partners participated in the study. On discovering the disease, the partners experienced emotional distress caused by the fear of losing their partners. Before this suffering, they demonstrated the ability to react: willingness to provide care, to comfort and to support the women in search of treatment, and together, seek to reverse adverse situations. They also showed sensitivity and recognized the intense emotional suffering of the women caused by the challenges faced throughout the course of treatment. Conclusion: The present study showed the intense suffering of the partners of women with breast cancer and their difficulty to ask for help. Thus, health professionals must be alert, addressing their needs throughout the treatment process to mitigate suffering.
Introdução: O câncer de mama é a principal causa de morte por câncer na população feminina e a segunda neoplasia mais frequente no mundo. A detecção precoce da doença, principalmente por meio da mamografia, é fundamental para reduzir o número de óbitos e melhorar o prognóstico. Objetivo: analisar o conhecimento de mulheres entre 35 e 69 anos cadastradas em Estratégias de Saúde da Família (ESF) de Alfenas – MG acerca do rastreamento do câncer de mama. Métodos: Estudo quantitativo, descritivo e transversal no qual foram aplicados questionários em uma população amostral, selecionada a partir do total de mulheres cadastradas em cinco ESF de Alfenas – MG, sendo entrevistadas 224 mulheres a partir de questionário estruturado. Os dados foram analisados utilizando-se a estatística multivariada por meio da técnica de componentes principais. Todos os critérios éticos foram respeitados. Resultados: As ações preventivas de rastreamento influenciaram na redução da mortalidade e melhor prognóstico da doença. A terapia de reposição hormonal está correlacionada com as alterações encontradas nas mamas. A idade da primeira menstruação mostrou-se correlacionada às mulheres que já estão na menopausa, características essas consideradas de risco para o desenvolvimento da neoplasia de mama. A regularidade com que as mulheres realizam a mamografia depende de seu conhecimento acerca deste exame, considerando-se a realização e prática do autoexame importante. Verificou-se que o uso de anticoncepcional está diretamente relacionado com a idade e o estado civil, além de alterações encontradas no exame clínico das mamas. Estar na menopausa esteve diretamente relacionado à realização de mamografia. Fatores de risco para câncer de mama como tabagismo, dieta rica em gordura e etilismo, além de estarem altamente correlacionados, apresentaram uma relação inversa com a realização da mamografia. Conclusões: o conhecimento acerca dos exames preventivos e da sua importância está relacionado com a regularidade com que as mulheres os realizam.
