IntroductionPrimary intracardiac tumors are rare and approximately 50% are myxomas. The majority of myxomas are located in the left atrium and have variable clinical presentation. We report a case of a large myxoma in the right atrium, which is an uncommon location for this type of tumor.Case presentationA 45-year-old Caucasian woman with a history of palpitation had dyspnea on great exertion and discrete weight loss. A cardiac evaluation showed splitting of S1. An echocardiogram showed a large mass in the right atrium, suggesting myxoma; chest computed tomography confirmed the diagnostic hypothesis. Our patient underwent surgical treatment with excision of a 10 cm multilobulated mass. She presented with supraventricular tachycardia during the operation. She was placed in the intensive care unit and her condition improved after the use of amiodarone. The diagnosis of myxoma was confirmed by histopathological study.ConclusionsIn this case report, we emphasize the rarity of large myxomas in the right atrium and the difficulty of differential diagnosis given their dimension and location.
The scientific concern of this dissertation is to make it evident the conditions that paved the way to make room to produce the school subject of Sociology during its recent process of being reintegrated in the high-school curricula between 1996 and 2008. I call this process come-to-be discipline. Looking back at this period, I conduct a socio-historical analysis in order to demonstrate how the space for the production of such discipline as a whole was made up and weaved, from the political project to the educative project, from curriculum to academic. Based on a sociological analysis and reflection, I have attempted to understand the practices and meanings that can be found in the roots of this process, a fact that that impelled me to investigate such scenario, its agents, its speeches, its events, and its policies. By means of this study, I sought to reach an in-depth comprehension about the social, political, educational, and academic process which the school subject of Sociology has gone through before becoming some sort of "timeless data in the so called school grammar". The reason for that lies in the search for a deeper understanding of a peculiar period in the history of the discipline, where not only it was reintegrated to the curriculum, but mainly it represented the resume of the academic and scientific interest about this topic, encompassing new patterns of dependency for the discipline and providing the circumstances for a new phase of its development.
Resumo Neste artigo, iremos apresentar e discutir o relatório Um ajuste justo: análise da eficiência e equidade do gasto público no Brasil (2017), produzido pelo Banco Mundial para conjecturar acerca da necessidade de um ajuste fiscal radical para sanar as finanças e os problemas orçamentários do país. O relatório em questão apresenta alguns argumentos e estratégias para a execução de um ajuste das contas públicas, em especial, para setores como saúde, educação, previdência social, dentre outros. Partindo de uma abordagem crítica acerca das recomendações do Banco para a educação básica pública brasileira, questionamos as diretrizes expressas no documento, que apontam para a necessidade de contingenciamentos e cortes no orçamento como forma de melhorar as contas e equilibrar a razão investimento/resultados. O relatório ampara-se em uma econometria que busca legitimar a todo custo a tese que vê na política fiscal contracionista o melhor caminho para intervir na educação básica pública. Ao longo do texto, discutimos se isso é factível considerando o nosso contexto histórico de subfinanciamento, onde, diferentemente de outros países mais ricos e menos desiguais, jamais alcançamos um patamar de investimentos suficientes para garantir o acesso, a permanência e a qualidade na educação básica.
Objective: To describe our experience with repair of coarctation of the aorta in adults using left heart bypass.Methods: From November 2007 to October 2009, eight adult patients with coarctation of the aorta underwent surgical repair under circulatory support using a left atrium to femoral artery bypass circuit, with graft interposition tube through left posterolateral thoracotomy. Five patients were female, with mean age of 31.5 ± 13.1 years. All patients had hypertension and others associated cardiovascular diseases.Results: There were no deaths or neurological complications. The mean surgical time was 308 minutes with mean left heart bypass and distal aortic clamping time of 73 and 65 minutes respectively. Postoperative bleeding was 696 ml in average. Six patients developed severe hypertension postoperatively requiring intravenous vasodilators. The mean length of stay was 9 days. A significant reduction of gradient blood pressure occurred. Resumo: Em sua condição atual a disciplina escolar de Sociologia pode parecer mais estável e consensual do que alguns anos atrás, no entanto, ainda persistem muitas dúvidas sobre seu projeto disciplinar. Sua recente consolidação como disciplina escolar tem produzido uma história curricular bastante oscilante em virtude das inovações educativas trazidas pelas políticas educacionais do período pós Lei 3.934/96. Fruto desse contexto emergem concepções de ensino e estratégias metodológicas para fomentar a produção da disciplina. Neste artigo apresento algumas ideias a partir de uma pesquisa sobre o que pensam os professores de Sociologia e os efeitos de suas representações nas práticas de ensino.Palavras-chave: Sociologia.
