Background: Medical records are files that contain identity, history, physical, laboratory, diagnosis and medical treatment of a patient recorded both in writing and electronically. The system for administering medical records starts from recording as long as patients get medical services, followed by organizing, storing and issuing medical record files from storage to serve requests / loans by patients or for other purposes. Currently health facilities are trying to replace manual medical record management into electronics. Klinik Pratama Romana Deli Serdang is one of the clinics in the Deli Serdang area that has management planning to change the implementation of a manual medical record unit that has been running into an electronic medical record. The research objective was to design an electronic medical record concept for the Pratama Romana Clinic. This type of qualitative descriptive researched with informants as many as 5 people. Data were collectioned through observation and interviews. The results of the study showed that the management of medical records required qualifications of officers with a background in D4 management of health information, make procedures from registration to reporting and coding according to the ICD and establishing record access rights. The conclusion was that the management of Pratama Romana Clinic already had high support in the implementation of electronic medical records until the application of the concept can be carried out in stages in accordance with the standards and needs.
In learning, interest from every child is needed so that they can absorb and understand every material presented by the teacher. However, in reality, children's interest in learning is often erratic, so their concentration is not focused. The purpose of this study was to determine the implementation of the Adiwiyata school concept for early childhood to increase the learning interests in 10 kindergartens throughout North Tapanuli Regency, with similar problems. The implementation of Adiwiyata was predicted incresing interest of early childhood in learning. This research was conducted in all kindergarten schools in North Tapanuli Regency which was carried out from April to October 2022 and used a mixed method approach, which is a mixture of qualitative and quantitative. Based on research that has been carried out in the field, the implementation of the adiwiyata school concept in North Tapanuli Regency Kindergarten is relatively good. This is based on the average value (mean) of the implementation of the adiwiyata school concept in the high category. Meanwhile, all students have a high interest in learning. Through this research, it is hoped that ECCE educators and education staff will make new innovations in the environment so that they can get the concept of adiwiyata to be achieved optimally
Nutirional methods of the elderly is most important. Changes in body function effect to concumption and absortion of nutrients. The elderly with nutritional problems, it is necessary to cure and prevent complications from the disease. The most problems often are is malnutrion and obesity, it caused due to changes in diet and activities after retirement. Goals : This study aims to determine the description of the nutritional status of the elderly in the Nurse Study Program area of STIKes Santa Elisabeth Medan Method : The present study is descriptif to asses the nutritional status of the elderly Gerontik Ners Nursing Area STIKes Santa Elisabeth Medan using Body Mass Index (BMI) by measuring body weight and height in 30 elderly people. Results: The results showed that the majority of elderly people in the Elderly category (60- 74) 56.4%, the elderly who had gout acid comorbidities were 63.3% and over nutrition status 53.4%. Conculsion : From the results of the study it was found that the majority of the elderly experienced over nutrition 53.4%, based on the study suggested that the Gerontik Nursing group could provide optimum for the elderly. optimum nutrition for the elderly.
Industrial sector has a high risk of work accidents. So, it is necessary to apply occupational health and safety management to monitor the work process. The aims of this study are to evaluate and analyze the risk of noise exposure in the production process at sanitary industries. The study was conducted in the sanitary ceramics industry which produces sanitary equipment, in the production area and other sanitary equipment. The method used was cross sectional design study, there was seven samplings location and 98 respondents to determine the perceptions of health risks. The results obtained that the highest noise is 101.9 dB (A) in the forging section, 92.6 dB (A) in the casting section, 91.7 dB (A) in polishing belt section, and 89.6 dB (A) in polishing section buff. According to The Government No. 13/2011 the noise quality standard for the industry at 80 dB (A). The highest risk of noise exposure is to complaints of headaches (OR = 2.44), high blood pressure (OR = 1.24), fatigue (OR = 2.21), hearing loss (OR = 1.99). Suggestions that can be given are the need for training and outreach to workers about the risks associated with specific work on 8 hours work noise exposure to avoid work accidents.
Elderly is the life stage of the individual, this phase of this life must be passed by every individual, health conditions at this stage is largely determined by the quality and quantity of nutrient intake. In addition to nutritional intake such as energy and protein intake, physical activity can also contribute to the state of nutritional status of the elderly. Pancur Batu Puskesmas Goals : Deli Serdang has posyandu elderly in every rural area working area of puskesmas. One of them is an elderly service posyandu unit in Tanjung Anom Village. This study aims to see the factors that affect the nutritional status of elderly in posyandu elderly village tanjum anom. Method : The research design used is cross sectional study approach. This study has a sample of 38 people, by way of taking purposive sampling. The tools used in data collection are questionnaire and Food Frequency Quetionnaire (FFQ). Results : The results showed that the majority of women (68.5%), unfulfilled protein intake 55.3%, 50% unmet energy intake, and physical activity 73.7%. The results showed that protein intake and physical activity factors were associated with nutritional status of the elderly, as evidenced by p = 0,004 and 0,003. Conclusion: The conclusion of the study was that low intake of protein and energy led to nutritional status of the elderly. It is recommended that family members living with the elderly can balance the intake of protein and energy that is tailored to the type of physical activity of the elderly everyday.
