Fraktur gigi anterior yang parah akibat trauma sering menyisakan sedikit struktur gigi yang menjadi tantangan dokter gigi untuk merestorasi. Retensi yang minimal memungkinkan terjadinya kegagalan restorasi, sehingga kebanyakan dokter akan lebih memilih melakukan pencabutan. Crown lengthening adalah prosedur pembedahan untuk mengekspose mahkota klinis yang tersisa sebagai usaha untuk meningkatkan retensi restorasi. Tujuan laporan kasus ini adalah untuk mengevaluasi hasil restorasi pasca dilakukan crown lengthening. Laporan kasus seorang perempuan usia 20 tahun mengalami fraktur gigi anterior maksila pasca kecelakaan yang dialaminya 2 minggu lalu. Gigi 21 fraktur dengan garis patahan horizontal 1/3 servikal mengenai pulpa. Status periodontal baik dan tidak ditemukan kelainan pada gambaran radiograf. Penatalaksanaan dilakukan scalling dan root planning pada regio gigi 21 sebelum pembedahan. Gingiva dipotong sedalam 2 mm pada seluruh permukaan menggunakan blade nomor 15 dilanjutkan penutupan luka bekas pembedahan dengan periodontal dressing. Perawatan saluran akar dilakukan dan diikuti dengan restorasi pasak fiber serta inti dengan resin komposit. Porcelain fused to metal diinsersikan sebagai restorasi mahkota akhir. Hasil restorasi akhir gigi 21 menunjukkan retensi yang memadai untuk restorasi mahkota porcelain fused to metal. Mahkota yang diinsersi dapat bertahan setelah 6 bulan observasi dan pasien merasa cukup puas dengan hasil perawatan. Kesimpulan dari kasus ini, prosedur crown lengthening dapat menunjang restorasi akhir yang retentif pada kasus fraktur gigi anterior yang kehilangan sebagian besar struktur mahkota.
Modern advances in dentistry have provided the opportunity for patients to maintain a functional dentition for lifetime. Hemisection is a conservative way of preserving tooth. This procedure is an endodontic treatment performed by removing of the one or more roots and existing crown structure to increase retention of the remaining teeth and to correct defected dental roots that are not possible to maintain. The purpose of this study was to report a case of hemisection management in teeth with bifurcation lesion. A 24-year-old female patient complained of pain and discomfort arising from the right mandibular first molar during mastication. 2 years ago, the patient underwent endodontic treatment. Based on the clinical examination, a deep cavity and failed restoration were found in tooth 46. Radiographic view showed a radiolucency on distal root, bifurcation and fracture line in mesio-cervical region. Endodontic retreatment was performed by cementing prefabricated fiber post on distal root canal and hemisection procedure was performed in the mesial root. The tooth was restored with splinted zirconia crown. Hemisection procedure and final restoration showed a good result. Hemisection was an alternative treatment in this case. Tooth can be preserved in economical way. Case selection is essential in performing sequences of procedure for the success of treatment.
Background: Gingival recession can cause esthetic problems, especially if it occurs in the maxillary anterior region. Gingival recession accompanied by endodontic complications requires a multidisciplinary approach to achieve successful therapy. Objective: To report the treatment of Miller’s class-III gingival recession using laterally stretched flap + connective tissue graft with frenotomy and apicoectomy in one visit. Case Report: A healthy 24-year-old man presented with chief complaints of open and painful gums on his left upper front tooth. After objective and radiographic examinations, the diagnosis of tooth 21 was Miller’s class-III gingival recession, that is, plaque and calculus with endo–perio lesions and grade 1 luxation. The treatment given included scaling, root planning, curettage, retreatment of root canal and root coverage therapy with frenotomy and apicoectomy. Results: The results obtained were partial root coverage of 71.4% and an increase in the thickness of keratinized tissue. Conclusion: Healing of periodontal tissue damage accompanied by endodontic lesions showed success and obtained stable treatment results with a multidisciplinary approach.
