AHORRO Y COSTES POTENCIALES DE UNA INTERVENCIÓN PARA REDUCIR LA NO-ADHERENCIA DE LOS PACIENTES CRÓNICOS AVANZADOS 2 IntroducciónLa adherencia al tratamiento es muy baja entre pacientes crónicos avanzados, implicando un gasto aún más alto del que ya es. Esta situación está empeorando debido al crecimiento exponencial del número de pacientes crónicos. Este estudio estima el beneficio neto de una intervención para la reducción de la no-adherencia, que consiste en monitorizar regularmente a los pacientes para evitar errores en la toma de las medicinas. MétodoSe hace un análisis coste-beneficio. Como coste se calcula el tiempo dedicado a la visita, y como beneficio la reducción del coste sanitario. Se usa la perspectiva social y como año de referencia 2014. También se hace un análisis de sensibilidad, utilizando diferentes valores para la variable eficiencia de la intervención. ResultadosCon una efectividad del 10%, el beneficio neto obtenido aplicando la intervención a una población de 7,5 millones de habitantes sería en 3.804 millones de euros. El beneficio neto, si se aplicara la misma intervención el año 2050, sería de 5.829 millones de euros debido al incremento de la población. Reduciendo la efectividad más de un 80% el beneficio aún sería muy importante. ConclusiónLa aplicación de esta intervención sería muy beneficiosa, no solo por el aumento de la salud de la población diana, sino también por la reducción del gasto sanitario.
Background Growing evidence shows the effects of psychotropic drugs on the evolution of dementia. Until now, only a few studies have evaluated the cost-effectiveness of psychotropic drugs in institutionalized dementia patients. This study aims to assess the cost-utility of intervention performed in the metropolitan area of Barcelona (Spain) (MN) based on consensus between specialized caregivers involved in the management of dementia patients for optimizing and potentially reducing the prescription of inappropriate psychotropic drugs in this population. This analysis was conducted using the Monitoring and Assessment Framework for the European Innovation Partnership on Active and Healthy Ageing (MAFEIP) tool. Methods The MAFEIP tool builds up from a variety of surrogate endpoints commonly used across different studies in order to estimate health and economic outcomes in terms of incremental changes in quality adjusted life years (QALYs), as well as health and social care utilization. Cost estimates are based on scientific literature and expert opinion; they are direct costs and include medical visits, hospital care, medical tests and exams and drugs administered, among other concepts. The healthcare costs of patients using the intervention were calculated by means of a medication review that compared patients’ drug-related costs before, during and after the use of the intervention conducted in MN between 2012 and 2014. The cost-utility analysis was performed from the perspective of a health care system with a time horizon of 12 months. Results The tool calculated the incremental cost-effectiveness ratio (ICER) of the intervention, revealing it to be dominant, or rather, better (more effective) and cheaper than the current (standard) care. The ICER of the intervention was in the lower right quadrant, making it an intervention that is always accepted even with the lowest given Willingness to Pay (WTP) threshold value (€15,000). Conclusions The results of this study show that the intervention was dominant, or rather, better (more effective) and cheaper than the current (standard) care. This dominant intervention is therefore recommended to interested investors for systematic application.
IntroductionPeripheral arterial disease (PAD) is a marker of cardiovascular morbidity, causing disability, loss of mobility and poor quality of life, manifesting clinically in the form of intermittent claudication (IC). Physical exercise increases the distance walked and improves quality of life. The aim of our study will be increased walking distance prolonging the time of onset of pain in patients with symptomatic PAD (IC).Methods and analysisThis study will be performed in Mataró Hospital’s vascular surgery service and School of Health Sciences, TecnoCampus. This population comes from 15 primary healthcare centres ofNorth Barcelona, Spain (450 000 inhabitants).This study will be a four-group parallel, longitudinal, randomised controlled trial, blind to analysis.The main primary outcome of this study will be the improvement in pain-free walking distance. Others primary objectives are and improvement in functional status, quality of life and Ankle-Brachial Index (ABI). Secondary outcomes will be the analysis of cardiorespiratory fitness, evaluation of muscle fitness, determine the maintenance of primary objectives at 6 and 12 months.We will be included 124 patients (31 per group). The changes of the outcome (Barthel, SF-12, VascQOL-6, ABI) of the three intervention groups vs the control group at 3, 6 and 12 months will be compared, both continuously (linear regression) and categorically (logistic regression). A person who has not performed at least 75% of the training will be considered to have not completed the intervention.Ethics and disseminationThe study will be conducted according to the Declaration of Helsinki . It was approved by the Ethics Committee of the Research Institute Primary Health IDIAP Jordi Gol (20/035 P),Barcelona 6 October 2020. Informed consent will be obtained from all patients before the start of the study. We will disseminate results through academic papers and conference presentations.Trial registration numberNCT04578990.
