Nine cases of botulism B with preponderant effects upon cholinergic autonomic innervation are presented. Blurred vision and dry mouth were constant symptoms. Impairment of salivary and lacrimal secretion were detectable for months. In the absence of clinical signs, electromyographic studies did not reveal neuromuscular involvement. Administration of antitoxin in the late course of such cases is not recommended, but guanidine can be used. Doubt is cast upon a new clinical entity called "acute autonomic neuropathy."
Detailed investigations after the second attack of a right-sided hemisymptomatology during high altitude mountaineering in an otherwise healthy man pointed to the diagnosis of migraine with aura (MA). Whereas the occurrence óf acute mountain sickness (AMS) and/or high altitude cerebral edema (HACE) and the possibility of a transient ischemic attack (TIA) or stroke should be considered as priorities, the diagnosis of MA should not be neglected.
Zusammenfassung. Bei einem Patienten mit VerschluB der A. carotis interna war an der Nasenwurzel, im Bereich des inneren Augenwinkels, eine sichtbar pulsierende Arterie vorhanden. Dieses Zeichen eines Kollateralkreislaufes kann fiir die klinische Diagnose eines Carotisverschlusses ntitzlich sein.
