Background:Glomus tumor is a common lesion of the subungual area of the hand fingers. However, glomus tumors located outside the hand region are rare and the diagnosis is often difficult due to their low incidence and lack of distinct clinical features in the physical examination. The presented article contains five cases of extradigital glomus tumors with a short review of the literature.Patients and Methods:Five cases of extradigital glomus tumor were included in the study. All lesions were purple colored subcutaneous nodules with sharp pain by digital palpation. All lesions were examined with ultrasound imaging were operated under local anesthesia using loupe magnification.Results:Among five patients, only one patient was female with a mean age of 35. Two lesions were located at the arm region, two at the crural region and one at the sternal area. The smallest nodule was 0.5 cm and the biggest lesion was 2 cm in diameter. In all the cases, the early postoperative period was uneventful without any surgical complication or acute recurrence. The postoperative 1st year examination of all patients revealed complete resolution of the pain and no recurrence was encountered.Conclusions:Glomus tumor should be kept in mind in the differential diagnosis of all painful subcutaneous lesions especially for those with purple reflection on the skin surface. In this manner, patients with extradigital glomus tumors may be diagnosed earlier and unnecessary and wrong treatments may be prevented.
SummaryIn this report, the gross and histopathological findings and the surgical treatment of a rarely seen case conjunctival dermoid was described in Belgian Malinois dog. A 6 month-old female Belgian Malinois dog, weighing 8 kg was referred to the University animal hospital with the complaint an abnormal mass on the right eye. In ophthalmic examination of right eye, epiphora and conjunctival hyperaemia were observed. Mass in the right conjunctiva at the temporal canthus was surgically removed. In microscopic examination, resected tissue revealed that dermoid was originated from conjunctiva. Sağ gözün oftalmik muayenesinde aşırı göz yaşı akıntısı ve konjunktival hiperemi belirlendi. Sağ göz konjunktivasının temporal açısında izlenen kitle cerrahi olarak uzaklaştırıldı. Mikroskopik muayenede, rezeke edilen kitlenin konjunktivadan köken alan bir dermoid olduğu anlaşıldı.
Amaç: Çene ve yüz kemiklerinin cerrahisi ile sıklıkla uğraşan plastik cerrahlar için gömülü dişlere yaklaşım, kendi başına taşıdığı riskler ve olası komplikasyonlar nedeniyle özellikle üzerinde durulması ve tarafımızca sahiplenilmesi gereken konular arasında gelmektedir. Bu çalışmamızda kliniğimizde tedavi görmüş 242 gömülü diş ve başka merkezlerde çekimi gerçekleştirilememiş yirmi bir diş kökü geriye dönük olarak değerlendirilmiş ve değişik tip ve konumdaki gömülü dişlere yaklaşımda bir algoritma oluşturulmuştur. Olası komplikasyonlar ve kurtarma girişimleri tartışılmıştır. Gereç ve Yöntemler:Kliniğimizde 2013-2015 yılları arasında tedavi görmüş 128 hasta geriye dönük olarak değerlendirilmiştir. Ortalama yaş 26 (aralık: 18-42) iken kadın/erkek dağılımı 39/89 olarak saptanmıştır. Hastaların on altısı bölgesel sinir bloğu ile geriye kalan hastalar genel anestezi ile ameliyat edilmiştir. Yüz yedi hastada diş çekimi uygulanırken, 21 hastada rezidüel kök çekimi uygulanmıştır. Seksen dokuz hastada kemik pencere açılması, diş çevresindeki kemiğin frezle açılması gibi kemik müdahaleleri yapılmış iken, geriye kalan hastalarda sadece mukoza insizyonları gerekli olmuştur.Bulgular: En sık saptanan başvuru nedeni bölgeye lokalize ağrı iken en sık ortaya çıkan komplikasyon şişlik ve ödem olarak saptandı. En sık çekim uygulanan diş üçüncü azı iken bunların çoğunluğu mandibulada gözlendi. Mandibulada lokalize dişlerin çekiminde, uzun dönemde en geç sekizinci aya kadar devam eden geçici alt dudak hipoestezisi altı hastada gözlenmiştir. Sonuç:Oluşturabildiği geri dönüşümsüz olabilen komplikasyonlar nedeniyle gömülü dişlere yaklaşımın, maksillofasiyal cerrahiyi de içinde barındıran plastik cerrahi disiplinince sahiplenilmesi gereken bir konu olduğu düşüncesindeyiz.Anahtar Sözcükler: Ektopik diş, gömülü diş, inferior alveolar sinir, maksillofasiyal cerrahi Objective: Impacted teeth are important for plastic surgeons that frequently perform maxillofacial operations because of their tendency to affect dental occlusion, and thus, cephalometric results. Moreover, severe complications are also caused by the tooth and its surgical removal. In this study, retrospective analysis of 242 extracted teeth and 24 extracted roots was performed and an algorithmic approach to different types and the localizations of impacted teeth was presented. Possible complications and salvage procedures were also discussed. Cutaneous angiosarcoma is an uncommon, potentially metastatic and highly aggressive vascular tumor that may arise as de novo or be associated with previous radiotherapy. A 70-year-old female with a solitary lesion on the nose was initially diagnosed as actinic keratosis. However, when the recurring lesion at the same region within 6 months was reexcised, the histological diagnosis was definitively established as well-differentiated angiosarcoma. This case was presented in order to increase awareness of this rare malignancy in an uncommon localisation, especially in the light of pathological findings. Moreover, occurrence of cutaneous angiosarcoma within a short period following a previous excision in a patient with a history of multiple nonmelanoma skin cancers was interestingly pointed out.
