In our study, the ARSA in combination with other ultrasound signs increased the risk for trisomy 21 by factor of 45, but the independent ability of ARSA as an isolated marker to predict fetal Trisomy 21 is unclear.
Abnormal fetal ear length is a feature of many chromosomal anomalies and aneuploidy conditions (1-3). In many studies, fetal ear length measurement has been identified as an additional marker for fetal chromosomal anomalies (4,5).Fetal ear length measurement is one that can be obtained during virtually all ultrasonographic examinations, from early in the second trimester of pregnancy until delivery. Moreover, ultrasound is a cheap and non-hazardous imaging modality. This provides the sonographer with a simple and effective means to assess fetal ear length, which is an additional marker for chromosomal anomalies.There have been a few studies that have given prenatal standards for fetal ear length measurement (5, 6). However, the majority of these studies have focused on Western populations. In this cross-sectional study, we measured the ear lengths of fetuses of Turkish women with normal pregnancies between 16 and 28 weeks gestation, in order to build an intrauterine ear length nomogram of our population. Our aim was to help to define normal and abnormal fetal ear lengths, particularly in the Turkish population.We hope that this specific nomogram will be better tailored to the Turkish population than the nomograms based on Western population data. MATERIAL AND METHODSEar length measurements were obtained prospectively in 389 singleton fetuses of gestations between 16 and 28 weeks. Amaç: Bu çal›flman›n amac›, klini¤imizde yap›lm›fl servikal serklaj olgular›n› retrospektif olarak analiz etmek ve iflleme ba¤l› olarak geliflmifl olan komplikasyonlar› de¤erlendirmektir.Yöntem: Nisan 2004 ile Aral›k 2012 tarihleri aras›nda Süleymaniye E¤itim ve Araflt›rma Hastanesi'nde servikal serklaj operasyonu geçiren hastalar çal›flmaya al›nd›. Hastalar›n özellikleri, ifllemin komplikasyonlar›, do¤umdaki gebelik haftas›, do¤um flekli ve postnatal komplikasyonlar retrospektif olarak incelendi.Bulgular: Yüz hastaya servikal serklaj suturu konuldu. Acil olgula-r›n oran›n›n %8.0 (n=8) oldu¤u gözlenirken, elektif olgular›n %68 (n=68), profilaktik olgular›n ise %24 (n=24) oran›nda oldu¤u sap-tand›. Ortalama do¤um haftas› 33.55±6.06 hafta idi. Do¤umlar›n %31'i (n=31) preterm, %69'u (n=69) ise term olarak gerçekleflti. Ortalama do¤um haftas› acil grupta 27.8±7.9 iken elektif grupta 33.9±5.8 ve proflaktik grupta 34.2±5.1 hafta idi. On (%10) hastada serklaj sonras› membranlar rüptüre (PPROM) oldu. Dokuz hastada membran rüptürü sonras› erken dönemde gebelik kayb› geliflirken, bir hastada üç hafta sonra gebelik kayb› geliflti. Bu has-talar›n birinde atefl ve koryoamniyonit geliflti. Do¤um s›ras›nda iki hastada servikal laserasyon olufltu. Komplikasyonlar incelendi¤inde; acil grupta komplikasyon gözlenme oran›n›n di¤er iki gruptan istatistiksel olarak anlaml› düzeyde daha yüksek oldu¤u gözlendi (p=0.005 ve p<0.1).Sonuç: Bizim sonuçlar›m›zda erken do¤um oranlar› nispeten yüksek seyrederken, di¤er komplikasyonlar (koryoamniyonit, atefl, servikal laserasyon) nadir görüldü. Acil serklaj grubunda, elektif ve profilaktik serklaj grubuna nazaran, erken do¤um oranlar› ve komplikasyonlar aç›s›ndan anlaml› bir yükseklik saptand›.
