Devletin sürekli artan göreviyle birlikte bu görevleri yerine getirecek insan kaynağı da artmakta ve çeşitlenmektedir. Kamu kurum ve kuruluşlarında çalışan kamu personeline özgü çalışma sistemlerinin bütünü insan kaynakları yönetimi ve o ülkenin veya o sektörün personel rejimini oluşturur. Devletin asli görevlerinden olan sağlık hizmetlerinin sunumunda da sağlık personeli sistemin en önemli bileşenini oluşturur. Bu çalışmada kamu personel yönetimi ve rejimi ile rejim içerisinde büyük yeri olan sağlık personelinin planlanması, Türkiye'deki bu konuyla ilgili politika dönüşümü ve bu politikalar ışığında oluşan istihdam çeşitliliği, nedenleri ve gereklilikleri incelenmiştir. Türkiye'de kamu sağlık sektöründe personel rejimiyle ilgili yapılan çalışmalar az sayıdadır. Kamu personel rejiminin önemli bir parçası olan sağlık personel rejiminde mevzuatlar ile düzenleme yapılmaya çalışılmış ancak zamanla yapı karmaşık, bölünmüş, parçalı bir hal almıştır. Ayrıca sağlık personel rejimini etkileyen aktörlerin ve konusu olan çalışanların unvan, yetişme/eğitim durumu, gelir düzeyleri, toplumsal konumları ve yaptıkları işler ile bağlı bulundukları kurumsal yapılar da oldukça farklılık gösterebilmektedir. Çalışmada literatür taraması yapılarak sağlık çalışanları analiz edilmiştir.With the ever-growing duties of state, human sources to fulfil these duties increase and vary, as well. The dimensions of working system peculiar to the civil servants working for state institutions and organisations are formed by an extensive personnel management and personnel regime of that country or sector. Health personnel also compose the most significant component of system in health service delivery which is one of the fundamental duties of state. It has been tried to regulate with legislations within personnel regime which is a substantial part of civil servant regime, however; the structure has become confusing, disunited and fragmental over time. In addition, the titles, backgrounds, levels of income, social positions, works to do and registered institutional structures of the actors affecting the health personnel regime and the personnel that are a subject of regime may also differ notably. In this study, personnel management and regime, planning of health personnel who have a substantial place within regime, political transformation In Turkey related to this subject, employment diversity created In the light of these policies, its reasons and necessities are analysed by reviewing the literature.
Associated with the market-oriented alterations of the government's role within economy following 1980s, government institutions and personnel have also had a change in the same direction. Our country has made a reform by preparing projects in this sense, as well. The most powerful of these studies is the Health Transformation Programme (HTP). Through HTP, the field of Health has been scrutinized fully, and different actions have been introduced, and nonconventional applications which are different from the previous structure in terms of organisation configuration, economy of health, monetary resources, application forms of service and personnel regime have been brought. The most important one of these different applications has happened on the side of health personnel. A large number of employment models have been set in different structures and legislations, and the most remarkable one of them has been the contractual management model. Under the guidance of HTP, the model of contractual management was adopted for the managers working at health institutions in rural areas with the dated 02/11/2011 Decree Law No: 663 concerning the Organisation and Duties of the Ministry of Health and Subsidiaries. Thanks to the adopted application of contractual management, career perception of the Ministry of Health has undergone a change and this system which has not been applied in Turkey before is precedential for other public enterprises, as well. In this study, the place, understanding and application of contractual management in health employment diversity which is an essential step for the sake of professionalising health management is analysed. Studies have been done regarding 663 numbered decree law throughout literature in Turkey. However, there are scarcely any studies about the contractual management research existing in the internal structure of health personnel when it is condensed into specific details. The contractual management in health system are analysed by making a literature and legal arrangements review for this study.
