BACKGROUND:Well-differentiated thyroid carcinoma (DTC) can be locally aggressive, invading aerodigestive tract. The rationale for aggressive surgical resection in this clinical setting is supported by a long-term local control with a positive impact on survival.CASE REPORT:A 60-year-old male patient was consulted by a digestive surgeon of unaware thyroid enlargement. Physical and imaging examination showed a suspect of thyroid malignancy. During surgery, we found that a tumour had invaded the anterior side of the trachea. Resection of three tracheal rings was performed, with end-to-end anastomosis. Surgical outcome regarding nervous preservation and parathyroid glands was good as well as cosmetic aspect. During one-year follow-up, no indication of tumour recurrence was found. The management of locally invasive DTC has been controversial yielding the palliative surgery modalities. Advances in surgical technique have given a new perspective of resection in a difficult case. This case report was managed by sleeve resection with end-to-end anastomosis which showed a satisfactory outcome functionally and cosmetically.CONCLUSION:Sleeve resection with primary reconstruction of the trachea is a simple one-stage procedure which can adequately address the problem of tracheal invasion by thyroid cancer.
Tujuan: Untuk menganalisa penggunaan sistem pembiayaan berdasarkan tarif INA-CBGs dalam penentuan rencana diagnosis, terapi dan monitoring agar pasien mendapat pelayanan terbaik dengan biaya seminimal mungkin. Salah satu yang perlu dipertimbangkan adalah peran pemeriksaan penanda tumor CA 15-3. Metode: Dilakukan telaah artikel untuk mengetahui peran dan manfaat penanda tumor CA 15-3, dibandingkan dengan biaya yang diperlukan untuk pemeriksaan pasien berdasarkan penelitian terbaru saat ini, guideine, pedoman nasional pelayanan kedokteran dan aturan tarif INA-CBGs. Hasil: Studi mengenai CA 15-3 memiliki hasil yang bervariasi sebagai pemeriksaan untuk menunjang diagnosis, penentuan staging, monitoring respon terapi, dan prognosis pasien. Berdasarkan guideline saat ini, CA 15-3 bukan merupakan penanda tumor yang utama pada kanker payudara dan hasil pemeriksaan CA 15-3 saja tidak dapat digunakan sebagai pedoman diagnosis, monitoring terapi, dan penentu pilihan terapi pada pasien. Biaya pemeriksaan CA 15-3 sebesar 285.000 rupiah cukup terjangkau untuk dilakukan, namun tetap merugikan bila hasil pemeriksaan tidak mempengaruhi diagnosis, monitoring, prognosis, dan pilihan terapi pasien. Simpulan: Bukti yang ada saat ini menunjukkan bahwa tidak diperlukan pemeriksaan CA 15-3 pada kanker payudara mengingat diagnosis, monitoring, prognosis dan terapi tidak dipengaruhi oleh hasil CA 15-3 yang diperoleh.
Tujuan: Untuk mengetahui pengaruh antara walking exercise programme (WEP) terhadap cancer related fatigue (CRF) pada pasien kanker payudara di RSUP Sanglah Denpasar. Metode: Penelitian randomized controlled trial group pretest-posttest design ini dilakukan pada seluruh pasien kanker payudara yang dirawat di RSUP Sanglah Denpasar pada tahun 2018, untuk dibedakan dalam kelompok kontrol dan kelompok perlakuan (yang menjalani WEP). Pengambil data dan pengolah data disamarkan mengenai kondisi randomisasi dan perlakuan. WEP dilaksanakan selama tiga minggu yang dilakukan sebanyak 3 kali dalam seminggu selama 30 menit. Pada kedua kelompok dilakukan penilaian skor brief fatigue inventory (BFI) sebelum dan sesudah perlakuan. Data kemudian dikumpulkan dan dilakukan analisis statistik dengan SPSS 25.0. Hasil: Penelitian awalnya terdiri dari 41 peserta di kelompok intervensi dan 41 di kelompok kontrol. Dua peserta dari kelompok kontrol mengundurkan diri dari penelitian. Pada analisis skor BFI setelah latihan, ditemukan bahwa skor BFI pada kelompok intervensi menurun secara signifikan dibandingkan kelompok kontrol (2,8±1,2 vs. 0,2±0,7; p<0,001). Dengan uji multivariat ANCOVA ditemukan bahwa setelah mengendalikan variabel perancu, aktivitas fisik WEP tetap dapat memberikan efek 2,5 kali lebih besar dibanding kontrol (p=0,001). Simpulan: Aktivitas fisik WEP menurunkan tingkat CRF pada pasien kanker payurada 2,5 lebih besar jika dibandingkan dengan kontrol sehingga direkomendasikan sebagai salah satu penanganan CRF pada pasien dengan kanker payudara.
Tongue SCC comprises 20% of all SCC involving oral cavity which increase in incidence worldwide. Glossectomy either sub-total or total is the mainstay of treatment for tongue SCC to achieve cure with optimum functionality. Various surgical might be used based on tumor burden. Reconstruction method for defect following the surgeries has always been challenging with variable options of flaps and variable outcomes. These case series reporting 4 cases of tongue SCC treated with glossectomy, neck dissection and standard therapy. Approaches used were lateral mandibulotomy, lower cheek and visor flap approach. Each treated with different reconstructive technique following the surgery. Case 1 was reconstructed with pectoralis major flap, case 2 with latissimus dorsi flap, case 3 with sternocleidomastoid flap and case 4 with supraclavicular island flap. Despite the well-known benefits and disadvantages for every above-mentioned method for defect closure, all these flaps in our reports gave satisfactory result in tongue mobility including articulation and swallowing, as well as the local aspect such as flap survival, wound healing, and no involvement of significant post-operative complication. In conclusion, anatomical, functional and aesthetical outcomes after glossectomy using different type of flaps are generally comparable.
