A 69-year-old male with a history of chronic obstructive pulmonary disease, esophageal cancer status post-resection with gastric conduit, subsequent anastomotic leak with fistula treated with esophageal stenting who presented to our tertiary care hospital as transfer with computed tomography (CT) (figure 1) evidence of multifocal pneumonia, pneumomediastinum, and pneumopericardium after receiving ceftriaxone and metronidazole.
