IntroductionIn Brazil there is a great demand for evaluation of the capacity to exercise civil rights of mentally ill patients, aiming their judicial interdiction as a protective measure. In Rio de Janeiro, the Public Attorney Office counts with a seven psychiatrists team, that among other functions, will evaluate the civil capacity of individuals, giving the district attorneys a technical psychiatric background to decide whether, or not, to demand a judicial interdiction.ObjectiveAnalyze five case reports in which district attorneys have demanded a civil capacity evaluation.MethodThis is a serial cases study of five patients, where each individual was assessed. The final psychiatric diagnosis was obtained with a psychiatric evaluation of the patients, the examination of their records or relatives interview.DiscussionThe five patients, each with a different diagnosis, were assessed individually. The civil incapacity is not an absolute state, but a relative situation, and the reasons and circumstances are important elements in forensic evaluation.ConclusionsInterdiction is not always the best, or even the most convenient solution to social, economic and legal problems occasioned by the absence or loss of insight or self-determination caused by disability or mental illness. It is vital to characterize the absence or loss of sense of judgment, self-determination or volition, and it is necessary to determine that such damage would be significant to the specific conditions of the familiar and socio-economic profile of each citizen.
Introduction: The essential characteristic of Selective Mutism is the consistent failure to speak in social situations where speech is expected, despite the capacity of speaking in familiar places or situations. The child usually refuses to speak at school, but speaks normally and well at home. Most cases start at preschool age and have transitory course. Psychotherapeutical interventions and selective serotonin reuptake inhibitors are considered first and second line treatments, respectively. Objectives: The purpose of this current report is to describe the successful treatment of selective mutism in a school-age boy. Methods: This is a case report about a multidisciplinary approach on a boy who stopped talking at the age of 3, and restarted talking by the time he was 8 years old, after 1 year of treatment. Results: This study showed the improvement of the patient after he changed from a group therapy only to a combined psychiatric + individual psychotherapy + phonoaudiological treatment + family orientation + educational work (school). Discussions: This case of Selective Mutism had better and faster results with a multifocused treatment than single therapies only.
Introduction: The project "RESTAURANDO ESPERANÇA" (Restoring Hope) aims to tackle social exclusion between children and adolescents, deriving from drug use, associated with problems such as physical and sexual violence, school failure, mental health disorders, lack of access to appropriate education, health, housing, etc.-conditions characteristic of selective and unequal societies such as the Brazilian one. Aims: The purpose of this current project is achieving adherence to the treatment of youth, promoting social inclusion, prevention and treatment against the use of alcohol and drugs, restoring their health and quality of life. Methods: The GEAL/UFF and IPHEM Health Group, part of this Project, attends to a public between 10 and 18 years old, living at social risk and/or using alcohol and drugs. The Pizzeria-School is a project that aims to train low-income young people. In order to increase attendance and to provide a longer stay of youths in the Living Center, the Health Group and the Pizzeria-School have associated and that resulted in a new concept of therapeutic approach. Results: This association resulted in a group called "Kitchen's Talk" with a surprising adhesion and full attendance. Conclusions: The "Kitchen's Talk" provides a less formal atmosphere and makes participants more comfortable during the group chat. At the end of the lesson, the encounter between the youth and their families, mediated by the therapeutic team, is celebrated with tasty pizzas, as a result of the work and the commitment of each individual, allowing the redefinition of values and the building of new family ties.
Introduction:In Brazil, the public health system is universal and regionalized. Currently, it is being restructured starting from the primary care system.In Rio de Janeiro there is a lack of children and adolescents mental health professionals, and there is a deficient system to attend the demand, without integration between health and educational systems and insufficient and inadequate psychology, pedagogy, speech therapy, occupational therapy and medical care.The city of Rio de Janeiro is divided in 10 Programmatic Areas (PA). The Child and Adolescent psychiatry of Piquet Carneiro Policlinic (PPC)/ UERJ covers, amongst three more units, the PA 2.2 of 371.120 habitants, amid those, 74.257 are under 15 years old (IBGE, 2010).Objectives:Analyze the Child and Adolescent Psychiatric demand in PPC/ UERJ in 2013, arising from the primary care system.Methodology:A sample of 50 patients that were evaluated in this policlinic referred from the primary care system were attended and directed through consultations. Those evaluations were made by psychiatrists and reports and counter-referrals were fulfilled. The patients in need of psychiatric care are being accompanied.Results:Out of the 50 patients, we analyzed gender, age, diagnosis, familial background, social-economical background, educational achievement and prescribed drugs.Conclusions:Among the analyzed patients, we found that the majority of the cases regard learning disabilities and family issues that could have better prognosis with an integrated and multidisciplinary care.The existing units covering Child and Adolescent Mental Health in the referred PA are not enough to provide the necessary care.
