The lack of qualified mental health nurses is at critical level with the problem likely to worsen as the aging mental health nursing workforce retires. This study investigates the career preferences of undergraduate nursing students by comparing preferences at the start, middle, and end of the Bachelor of Nursing program. The comparison of the cohorts gave an indication of the change in preferences over the intervening years. It replicates research completed in 1992, 1997, and 2001, and develops a profile of nursing career preferences and the rationale underpinning those preferences in a cohort of students (n = 150) who began their Bachelor of Nursing studies in 2007 and completed in 2009. The main findings included that, like the previous studies, mental health nursing is one of the least desirable career choices for most nurses at the start of their course and remains so as they approach graduation. The reasons change but the outcome remains the same. The current system of using the Bachelor of Nursing award to produce mental health nurses in Australia does not encourage nurses to consider a career in mental health nursing. Which begs the question: where will mental health nurses in the future come from?
Dementia care mapping (DCM) is a popular method for evaluating the quality of care and well-being of people with dementia in formal care settings. Keywords and thesaurus searches were conducted between 1992 and June 2001 using a range of bibliographic databases. Studies that had specifically examined the efficacy of DCM or, had used DCM as the main outcome measure, were included in the review. Nine studies met the inclusion criteria and were evaluated in this review. The review highlights some methodological limitations in the DCM studies to date, including sampling bias, inadequate sample size, short evaluation periods and a lack of consideration of the confounding variables commonly associated with dementia. The evidence presented for DCM suggests that it has good face validity and reliability. However, other aspects of validity remain less convincing and it can only be regarded as a moderately valid instrument. Whilst its theoretical background makes DCM particularly appealing to nurses wishing to improve the quality of their care, it is a very time-consuming method and requires considerable investment in terms of nursing resources. Consequently, it is essential that more scientifically based studies are conducted to enable other aspects of validity to be developed, thus enhancing the value of DCM as an outcome measure of nursing care.
Primary care is developing rapidly with significant impacts on the nursing team. Such changes have brought inter-professional team-working into sharper focus, particularly community care and collaborative working. This paper: examines the nursing roles within a general practice; describes the perspectives of service users; identifies areas of change; clarifies core and specialist skills; defines new roles among the primary health care nursing team; proposes a new model of working; and identifies appropriate education. The project was set in a general practice in south-west England and used an action research methodology. The objectives were to create a change in practice and to develop and refine existing theory to underpin nursing roles. Throughout the research regular team meetings allowed reflection and discussion about research findings and progress. Data were collected from multiple sources, including team workshops, patient focus group interviews, and individual interviews with GPs, practice managers and area managers. Reflective diaries and a patient survey were also used. The analysis of the quantitative and qualitative data collected from patients formed a basis for practice development and facilitated the team's reflection on the areas of change. Overall high satisfaction with services and care was expressed in the patient interviews and the questionnaire. The themes from the data highlighted areas important for patients and helped in shaping the new roles and responsibilities for team members. Regarding the team perspective, the data indicated many areas that could be considered for development. The community nursing team decided to concentrate on three key areas: child health, leg ulcer management, and cardiovascular health. The research concludes that action research presents some problems and challenges but is a useful approach to developing team-working in primary health care.
