Tuberculosis (TB) is a leading public health concern in Indonesia. It ranks second on the list of high-burden TB countries. In West Sumatra, 47% of TB cases are undetected, late diagnosed, and received incomplete treatment because of low-level awareness and knowledge and stigma, especially among the hardest to reach populations. The study aims to identify the best communication channel to reach those who live in vulnerable and remote areas. This study was a qualitative study applying in-depth interviews to the informal leaders, health officers, cultural artists, and religious leaders across districts in Mentawai and Solok Districts, which are remote and had the lowest case detection rates compared with other districts. The questionnaire was prepared with the perception of the channel to identify TB cases. The data were analyzed using the content analysis technique. Involving religious and informal leaders and using traditional music as a communication channel improved the population's awareness of TB symptoms and access to TB testing and treatment, as well as reduced TB-related stigma. This study found that the cultural and religious contexts play a major role in health communication on TB control for hard-to-reach populations in West Sumatera, Indonesia.
AbstrakTuberkulosis masih merupakan masalah kesehatan di Indonesia karena prevalensi yang masih tinggi,i terutama di negara berkembang. Karena penyebarannya yang tinggi, maka perlu diketahui bagaimana profil penderita penyakit ini agar penularannya bisa diminimalkan. Tujuan dari penelitian ini adalah untuk mengetahui profil penderita tuberculosis paru BTA positif yang berobat di Balai Pengobatan Penyakit Paru (BP4) Lubuk Alung periode 1 Januari 2012 -31 Desember 2012. Penelitian ini bersifat deskriptif retrospektif. Instrumen yang digunakan adalah data dari rekam medik di Balai Pengobatan Penyakit Paru (BP4) Lubuk Alung sejak 1 Januari 2012 -31 Desember 2012. Populasi yang ada seluruhnya dijadikan subjek penelitian. Kemudian dilakukan pencatatan dari beberapa variabel yang diteliti. Hasil penelitian ini menunjukkan bahwa dalam periode tersebut jumlah penderita yang berobat ke BP4 Lubuk Alung adalah 19.440 orang, sebanyak 3.224 orang diantaranya suspek. Penderita BTA (+) 1.109 orang. Jenis kelamin laki-laki (70,8%) lebih banyak dari perempuan. Usia terbanyak adalah 21-30 tahun (23,2%). Daerah asal terbanyak adalah Kab. Padang Pariaman (29,4%). Derajat kepositifan BTA sputum terbanyak berupa positif tiga (+3) adalah (44,2%). Tipe penderita terbanyak merupakan penderita kasus baru sebanyak (91,7%). Keluhan terbanyak yang dirasakan ketika berobat adalah batuk (99%). Sebanyak (13,4%) memiliki penyakit penyerta selain tuberkulosis. Riwayat penggunaan obat sebelumnya sebanyak (11,3%). Sebanyak (99%) dirujuk ke puskesmas dan unit pelayanan kesehatan terdekat. Berdasarkan pendataan profil penderita TB Paru BTA Positif bisa dilihat paling banyak adalah derajat (+3) dan dirujuk ke unit pelayanan terdekat. Due to the speed of spread, it is necessary to know the profile of people who suffer from this disease, so the transmission can be minimized. The purpose of this study was to determine the profile of positive acidfast-bacilli (BTA) in pulmonary tuberculosis patients who seek treatment at Medical Center for Pulmonary Diseases (BP4) Lubuk Alung during the period 1 January 2012-31 December 2012.This is a descriptive retrospective study by taking the data from medical records in BP4 Lubuk Alung. Using the enterety of the population. The results of this study indicate that in this period the number of people who went to BP4 Lubuk Alung were 19.440 people, 3.224 of them suspected tuberculosis. Patients with BTA (+) was 1.109 people. We found male 70.78%. Most are 21-30 years of age 23.2%. The area of origin mostly from Kab. Padang Pariaman 29.4%. The degree of sputum smear positivity mostly positive three (+3) was 44.2%. Type of most patients are people with new cases 91.7%. Most complaints was cough 99%. Abstrak Tuberkulosis (TB) paru masih menjadi masalah utama kesehatan global di dunia. Pada tahun 2011, Indonesia berada di posisi keempat dengan jumlah penderita TB terbanyak di dunia. Dalam upaya penanggulangan TB, Indonesia telah mengadopsi strategi DOTS sejak tahun 1995. Berdasarkan laporan tahunan Dinas Kesehatan Kota Padang tahun 2011, angka keberhasilan pengobatan di Puskesmas Padang Pasir yaitu 71,43% dan angka ini belum mencapai target nasional. Penelitian ini bertujuan untuk mengetahui hubungan pelaksanaan strategi DOTS dengan hasil pengobatan TB paru di Puskesmas Padang Pasir Kota Padang. Jenis Penelitian ini adalah analitik dengan desain cross sectional study. Data dikumpulkan melalui wawancara kepada responden menggunakan kuisioner yang kemudian di analisis melalui uji Chi-Square. Hasil uji statistik Chi-Square menunjukkan adanya hubungan antara pelaksanaan komitmen oleh petugas kesehatan (p-value : 0,000), pelaksanaan pemeriksaan dahak (p-value: 0,005, ketersedian OAT (p-value : 0,002) dengan hasil pengobatan TB paru. Hasil uji statistik pada peranan PMO (p-value : 0,185) dan pencatatan pelaporan penderita TB paru (p-value 0,184) menunjukkan tidak terdapat hubungan dengan hasil pengobatan TB paru. InTB controlefforts, Indonesia hasadopted theDOTS strategysince 1995.Based on the annual report of Padang City Health Department in 2011, the treatment success rate in Padang Pasir Health Center is 71.43% and this figure has not reached the national target. The study aims to determine the relationship implementation of the DOTS strategy with pulmonary TB treatment success in health centers Padang Padang Pasir.This type of study design was cross-sectional analytic study. Data were collected through interviews with respondents using a questionnaire which was then analyzed by chi-square test. The results of the chi -square statistical tests showed that the implementation of commitments by health workers (p -value: 0.000), the implementation of sputum examination (p -value : 0.005), availability of OAT (p -value : 0.002) have a correlation with the results of treatment of pulmonary tuberculosis . While results statistical tests on the role of the PMO (p -value : 0.185) reporting and recording of pulmonary TB patients (p-value : 0.184) showed there was no correlation with the results of treatment of pulmonary tuberculosis.
AbstrakPenyakit Paru Obstuktif Kronik (PPOK) adalah penyakit dengan karakteristik keterbatasan saluran napas yang tidak sepenuhnya reversibel dan dapat dicegah. Indonesia merupakan salah satu negara berkembang yang memiliki jumlah perokok aktif yang tinggi. Tujuan penelitian ini adalah untuk mengetahui gambaran derajat merokok pada penderita PPOK di Bagian Paru Rumah Sakit Umum Pusat Dr. M. Djamil Padang. Ini adalah penelitian deskriptif retrospektif dengan menggunakan data rekam medik penderita merokok dengan yang dirawat di Bagian Paru Rumah Sakit Umum Pusat Dr. M. Djamil. Data sekunder diambil dari rekam medik penderita Penyakit Paru Obstruktif Kronik (PPOK) yang dirawat di Bagian Paru Rumah Sakit Umum Pusat dari 1 Januari 2013 sampai 31 Desember 2013. Data diolah secara manual dan disajikan dalam bentuk tabel distribusi frekuensi. Sebanyak 20 subjek didapatkan nilai indeks brinkman berat sebanyak 15 orang. Setelah dilakukan uji statistik didapatkan hubungan yang signifikan antara derajat merokok dengan derajat keparahan PPOK (p = 0,033) dan korelasi yang kuat antara keduanya (r = 0,577). Terdapat hubungan yang signifikan dan korelasi yang kuat antara derajat merokok dengan derajat keparahan PPOK.Kata kunci: merokok. indeks Brinkman, PPOK AbstractCOPD is a disease with airway limitation characteristic. That is not fully reversible and can be prevented. Smoking is the leading cause of COPD. Indonesia is one of the developing countries that have a high number of active smokers. The increase in prevalence is also occurred in the province of West Sumatra, which increased from 30.2% in 2007 to 38.4% in 2010. The objective of this study was to describe the degree of smoking in patients with COPD in Pulmonary Section General Hospital Dr. M. Djamil. This study used a retrospective descriptive design using medical record data of smoker patients with Chronic Obstructive Pulmonary Disease (COPD) who were treated in the Pulmonary Section General Hospital Dr. M. Djamil. The data was taken from the secondary status of patients with Chronic Obstructive Pulmonary Disease (COPD) who were treated in Pulmonary Section General Hospital from January 1, 2013 to December 31, 2013 that recorded in the medical record. Data processed manually from the status of COPD patients and were presented in the form of frequency distribution table. From 20 subjects obtained that 15 samples were classified to severe Brinkman index. Statistical tests found a significant relationship between the degree of smoking with COPD severity (p = 0.033) and a strong correlation between the two (r = 0.577). There is a significant relationship and strong correlation between the degree of smoking with COPD severity.Keywords: smoking, Brinkman Index, COPD
Abstrak Indonesia sebagai negara keempat terbesar didunia dalam jumlah penderita tuberkulosis (TB) paru. Pada tahun 2011, di wilayah kota Padang terdapat sebanyak 108 kasus TB paru per 100.000 penduduk. Kasus TB paru banyak ditemukan di wilayah puskesmas Seberang Padang yaitu sebanyak 46 kasus. Tujuan penelitian ini adalah untuk mengetahui hubungan tingkat kepatuhan berobat penderita TB paru dengan perilaku kesehatan, efek samping Obat Anti Tuberkulosis (OAT) dan peran Pengawas Menelan Obat (PMO) dalam pengobatan fase intensif di puskesmas Seberang Padang. Desain penelitian ini adalah analitik cross sectional dengan menggunakan kuesioner dan wawancara observasional. Populasi dalam penelitian ini adalah seluruh penderita TB paru periode September 2012-Januari 2013 yaitu 34 orang. Analisis statistik yang digunakan adalah chi-square. Hasil penelitian menunjukkan bahwa terdapat hubungan tingkat kepatuhan berobat penderita TB paru dengan perilaku kesehatan (p=0,00) dan peran PMO (p=0,00), tetapi tidak terdapat hubungan dengan efek samping OAT. Disarankan untuk lebih aktif dalam melakukan penyuluhan mengenai penyakit TB paru kepada penderita TB paru dan keluarganya. In 2011, the city of Padang has 108 cases in 100.000 population. Pulmonary TB cases are founded in the region of Public Health Center (PHC) of SeberangPadang by 46 cases. The objective of this study was to determine the correlation between the compliance treatment of pulmonary TB patients and health behaviors, side effects of drugs and the role of a treatment supporter in the intensive phase of treatment at PHC of Seberang Padang. The design of this study was cross sectional's analitic that used quetionnaires and interviews with observational. The population were all patients of pulmonary TB in period September 2012-January 2013 is 34 peoples. Statistical analysis has used chi-square. The results showed, there is a correlation between the compliance treatment of pulmonary TB patients with health behaviors (p=0,00) and the role of the PMO (p=0,00), but there are no correlation with the side effects of drugs. Suggested to be more active in the education of pulmonary TB disease to pulmonary TB patients and their families.
