Objective: We studied the significance of BsmI restriction enzyme polymorphism of the vitamin D receptor (VDR) gene and the XbaI and PvuII polymorphisms of the estrogen receptor (ER) gene in patients with type 2 diabetes n 49Y android type obesity with normal carbohydrate metabolism n 29 and healthy controls n 138X Methods: The distribution of genotypes in the study groups, as well as their relationship to fasting and 1 h postprandial serum C-peptide levels were analyzed. Results: Postprandial serum C-peptide levels of BB genotypes were significantly higher in the diabetes and obese groups 6X18^5X09 ngaml compared with other genotypes 2X71^2X45 vs. 1X721X97 ngamlY respectively, P 0X05X Among patients with type 2 diabetes and obese subjects, the XX allelic variant of the ER gene was more frequent P 0X00015X Postprandial C-peptide levels of subjects exhibiting XX genotype were significantly lower compared with those with Xx genotype 1X67^2X16 vs. 3X8^3X72 ngamlY P 0X021X The BBXx allelic combination of the VDR/ER receptor genes was less frequent in diabetic patients than in healthy subjects or in obese patients. The BBXx genotype was associated with significantly elevated postprandial C-peptide levels in all subjects compared with other combinations 9X65^3X14 vs. 1X35^2X82 ngamlY P 0X003X No difference was found in the distribution of the PvuII polymorphism of the ER gene or in the association with the C-peptide levels among study groups. Conclusion: Polymorphisms of the VDR/ER receptor genes might play a role in the pathogenesis of type 2 diabetes by influencing the secretory capacity of b-cells.
This study assessed persistence and compliance with anti-osteoporosis therapies, and associations between compliance and clinical outcomes (fracture, fracture-related hospitalization and death), in Hungarian women with postmenopausal osteoporosis. The study used the Hungarian National Health Insurance Fund Administration database and included women with PMO aged at least 50 years, for whom a prescription for anti-osteoporosis medication had been filled between 1 January 2004 and 31 December 2013 (index event). Persistence (prescription refilled within 8 weeks of the end of the previous supply) was evaluated over 2 years; good compliance (medication possession ratio ≥ 80 %) was evaluated at 1 year. Associations between compliance and clinical outcomes (data collected for up to 6 years) were assessed with adjustment for baseline covariates. A total of 296,300 women met the inclusion criteria (524,798 index events). Persistence and compliance were higher for less frequent and parenteral therapies (1- and 2-year persistence: half-yearly [parenteral] vs. daily/weekly/monthly [oral and parenteral], 81 and 38 % vs. 21-34 and 10-18 %, respectively; parenteral vs. oral, 75 and 36 % vs. 32 and 16 %; good compliance: half-yearly vs. daily/weekly/monthly, 70 vs. 24-39 %; parenteral vs. oral 78 vs. 36 %). Good compliance significantly reduced the risks of fracture, fracture-related hospitalization and death (relative risk vs. non-compliance [95 % confidence interval]: 0.77 [0.70-0.84], 0.72 [0.62-0.85] and 0.57 [0.51-0.64], respectively; P < 0.01). Improving compliance through long-interval parenteral therapies may result in clinical benefits for patients.
Significant differences observed in gene expression profiles of osteoporotic and non-osteoporotic human bone tissues provide further insight into the pathogenesis of this disease. Characterization of the differences between osteoporotic and non-osteoporotic bones by expression profiling will contribute to the development of diagnostic tools in the future.
Abstract:Many technologies have appeared in agriculture to reduce the harmful effects of chemical use. One of these technologies is precision farming technology. Precision farming technology should not be considered as only the latest plant production technology or only a new agro-management tool. It is achieved only when the results of electronics and IT equipment are realized in the variable rate treatments zone-by-zone. The advantages and disadvantages of this technology highly depend on the heterogeneity of soil, the knowledge and attitude of the manager and the staff. This is the reason why opinions about the technology effects are so wide. This paper shows the results of the investigation based on interviews about the adoption and knowledge of precision farming technology among Hungarian crop producers. This technology is mostly used by farms over 300 hectares with young farmers. The most characteristic elements were precision fertilization and tractor guidance. The survey examined three groups of farmers with respect to whether they apply precision farming elements or not. We refer to them as "users", "planners" and "non-users". According to the survey, the opinions of the "user" and the "non-user" groups of farmers are not significantly different regarding the impacts of precision farming technology (the main advantages were the change in yield quantity, chemical usage and income). Furthermore, the opinions of the farmers regarding the changes in variable costs resulting from the adoption of precision farming technology were also examined (measured in percent). According to the opinion of the "user" group of farmers, the highest cost savings occurred in fertilizer and herbicide costs.
