This observational report describes a new case of SAVI, a recently defined monogenic inflammatory phenotype, that exemplifies an emerging group of disorders related to primary upregulation of type I interferon signaling.
DEAR EDITOR, Transient acantholytic dermatosis (TAD), also known as Grover disease (GD), is a papulovesicular polymorphic eruption more commonly reported in elderly white men. The main histological feature is an epidermal acantholysis associated with different degrees of dyskeratosis. The aetiology and pathogenesis remain unclear, but several local or systemic, potentially triggering conditions have been linked with this entity, including high body temperature, sweating, acute ultraviolet exposure, drug intake or internal neoplasms. Recently, a higher than expected incidence of TAD has been described with the use of BRAF inhibitors in metastatic melanoma but the coexistence of an underlying malignancy, a condition classically associated with TAD, might represent a confounding factor.
Study design: A case of thoracic spinal cord compression due to tumoral calcinosis (TC) is reported. Setting: Galiza, Spain. Case report: A 59-year-old man was admitted to our hospital with a 2-month history of gradual leg weakness and sensory deficit. The neurological examination revealed paraparesis with T12 sensory level. Magnetic resonance imaging (MRI) showed an extradural right posterolateral mass at T11-T12 level, resulting in a marked spinal cord compression. He underwent T11-T12 laminectomy and mass excision. Histological examination finally led to the diagnosis of TC. Conclusion: TC is an uncommon cause of mass lesions of the spine. Since there is no typical spine TC MRI appearance, the final diagnosis is done by histological studies. TC should be considered in differential diagnosis of spinal cord compression and constitutes a treatable cause of paraparesis.
