A randomly selected sample of 3494 children evenly representing a total population of 37 391 schoolchildren aged between eleven and twelve was screened for idiopathic scoliosis by the bending test. Ten per cent of the children showed clinical evidence of scoliosis and 6.4 per cent had positive radiological findings. Small curves were equally distributed between the two sexes, while large ones were more common in girls. Children with fair hair and blue eyes showed a higher percentage of positive findings than children with a dark complexion. A group of 112 children with curves between 7 and 16 degrees were followed up for an average of nineteen months. The pattern and evolution of these curves showed their close relationship to established scoliosis. Potential factors of instability and deterioration of the curves were evaluated.
Idiopathic scoliosis (IS) is known to result in lung volume and pulmonary compliance reduction. Boston brace treatment of IS is an additional factor causing restrictive respiratory syndrome due to external chest wall compression. Nevertheless, the immediate effect of Boston bracing on the pulmonary compliance of scoliotic patients has not been studied systematically. Spirometric and plethysmographic lung volumes, static lung compliance (C(ST)(L)) and specific lung compliance (C(ST)(L)/functional residual capacity) of 15 scoliotic adolescents (14 females and 1 male, of mean age 14.1+/-1.67 years, with mean Cobb angle 24.1 degrees+/-7.88 degrees) were recorded twice, in a random sequence: once without the Boston brace (nBB) and once immediately after wearing the brace (BB). Our findings showed that bracing reduced vital capacity, residual volume, functional residual capacity (FRC), total lung capacity, and forced expiratory volume in 1s in a proportional and significant way (P < 0.001). C(ST)(L) was also significantly reduced (P < 0.001), but C(ST)(L)/FRC remained unaltered. All BB and nBB indices were highly correlated. We concluded that Boston bracing in IS patients results in an immediate, predictable, and uniform reduction of lung volumes and pulmonary compliance. The reduction of C(ST)(L) under bracing conditions was related to the decrease of lung volume; the C(ST)(L)/FRC remained unaltered.
An epidemiological survey of idiopathic scoliosis derived by school screening in Greece has shown a three-fold rise in prevalence rate from 1% in 6-year-olds to more than 3% in 15-year-olds. Moderate curves (with a Cobb angle of 10° to 19°) are the most common curve magnitude encountered in both boys and girls. Typical curves (right thoracic, left lumbar, or right thoracic left lumbar double structural configurations) become relatively more prevalent with rising curve magnitude, while atypical curve patterns (left thoracic, right lumbar, or left thoracic right lumbar double structural configurations) reciprocally diminish. Growth is clearly an important environment in which curves progress and peak prevalence rates occur at the ages of 11 years and 13 years. Although it is not possible to prognosticate about the individual case, attention to these characteristics derived from epidemiological surveys is useful in assessing future curve behavior.
