Background The People’s Republic of China (P. R. China) has made significant progress on schistosomiasis control. Among the 12 provincial-level administrative divisions (PLADs) with schistosomiasis endemic in P. R. China, Guangdong, Shanghai, Fujian, Guangxi and Zhejiang PLADs (following as five PLADs) had successively eliminated schistosomiasis during 1985–1995. However, consolidation of the schistosomiasis elimination in these five PLADs remains challenging. In the current study, we sought to understand the epidemic situation in these post-elimination areas and their surveillance capabilities on schistosomiasis. Methods Annual data reflecting the interventions and surveillance on human beings, cattle and snails based on county level from 2005 to 2016 were collected through the national schistosomiasis reporting system and the data were analyzed to understand the epidemic status of schistosomiasis in the five PLADs. A standardized score sheet was designed to assess the surveillance capacity for schistosomiasis of selected disease control agencies in five PLADs and ten counties. Assessment on surveillance capacity including schistosomiasis diagnostic skills, identification of snails’ living and infection status and knowledge about schistosomiasis and its control were made. Descriptive analysis was used to analyze the epidemic status and evaluation results on surveillance capacities. Results The assessments showed that no local cases in humans and cattle or infected snail were found in these five PLADs since 2005. However, from 2005 to 2016, a total of 221 imported cases were detected in Zhejiang, Shanghai and Fujian, and 11.98 hm2 of new snail habitats were found in Zhejiang, Shanghai and Guangxi. In addition, snail infestation reoccurred in 247.55 hm2 of former snail habitats since 2011. For the surveillance capacity assessment, the accuracy rate of IHA and MHT were 100 and 89.3%, respectively. All participants could judge the living status of snails accurately and 98.1% on the infection status of snails. The accuracy rate of the questionnaire survey was 98.0%. Conclusions Elimination of schistosomiasis was consolidated successfully in five PLADs of P. R. China due to effective and strong post-elimination surveillance. Comprehensive consolidation strategies should be focused on the elimination of residual snails and the prevention of imported infection sources to consolidate the achievements of schistosomiasis control.
The E3 deubiquitinating enzyme ubiquitin-specific proteolytic enzyme 21 (USP21) plays vital roles in physiological activities and is required for Treg-cell-mediated immune tolerance. Using a murine model infected with Schistosoma japonicum, we observed that there were more cercariae developed into adults and more eggs deposited in the livers of the USP21fl/flFOXP3Cre (KO) mice. However, immunohistochemistry showed that the degree of egg granuloma formation and liver fibrosis was reduced. In USP21fl/flFOXP3Cre mice, levels of IFN-gamma, IL-4, anti-soluble egg antigen (SEA) IgG and anti-soluble worm antigen preparation (SWAP) IgG increased in blood, as determined using ELISAs and multiplex fluorescent microsphere immunoassays, while the levels of IL-10, lL-17A, IL-23, IL-9, and anti-SEA IgM decreased. In addition, the levels of the USP21 protein and mRNA in the liver and spleen of KO mice decreased. We further observed increased Th1 responses amplified by Tregs (regulatory T cells) and compromised Th17 responses, which alleviated the liver immunopathology. We speculated that these changes were related to polarization of Th1-like Tregs. Our results revealed the roles of USP21 in Treg-cell-mediated regulation of immune interactions between Schistosoma and its host. USP21 may have potential for regulating hepatic fibrosis in patients with schistosomiasis.
BACKGROUND The injury degree of the femoral head is a significant predictor of femoral neck fracture (FNF) incidence in patients with femoral head fractures (FHFs). However, the exact measurement methods have yet been clearly defined. OBJECTIVE To design a new measurement for the injury degree of the femoral head and investigate its association with FHFs with FNF. METHODS A consecutive series of 209 patients with FHFs was assessed regarding patient characteristics, computed tomography images, and rate of FNF. New parameters for injury degree of femoral head, including percentage of maximum defect length (PMDL) in 2D parameter and percentage of fracture area (PFA) in 3D parameter, were respectively measured. Reliability tests for all parameters were evaluated in 100 randomly selected patients. The sensitivity, specificity, likelihood ratios, and positive and negative predictive values for different parameter cut-off values were employed to test the diagnostic accuracy for FNF prediction. RESULTS Intra- and inter-class coefficients for all parameters were ≥ 0.887. The results of logistic regression analysis showed that average PMDL across all three planes and PFA were the significant predictors of FNF (p < 0.05). Areas under curves (AUCs) of all parameters were ≥ 0.719 (p < 0.05). The cutoff values of the average PMDL across all three planes and PFA were 91.65% and 29.68%. The sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, predictive positive value and negative predictive value of 2D (3D) parameters were 91.7% (83.3%), 93.4% (58.4%), 13.8 (2.0), 0.09 (0.29), 45.83% (10.87%) and 99.46% (98.29%). CONCLUSIONS The new measurement for injury degree of femoral head appears to be reliable and indicates strong emergence of femoral neck fracture in patients with femoral head fractures. 2D and 3D parameters in FHFs have been proposed in this study, which were a feasible adjunctive diagnostic tool in identifying FNFs.
