Non-Steroidal Anti-Inflammatory Drugs are frequently prescribed in medical practice, are one of the most widely used drug classes in the world and carry a low but tangible risk of causing Acute Kidney Injury, electrolyte imbalances and increased blood pressure. However, studies showing the long-term effects of NSAID use on the development of CKD are still minimal, as such studies routinely exclude people with CKD and rarely report renal outcomes. The present study aimed to report the importance of using this pharmacological class with caution. A descriptive, qualitative study of current Literature Review was carried out to prepare the work. Was the risk of ARI associated with the use of NSAIDs in patients with CKD evaluated? The Medline database (via PubMed) was used, with preference given to the most current full-text articles, such as from 2014 to 2021. Only articles in English were selected. The results of the literature review showed that current exposure to NSAIDs was associated with an approximately 1.5fold increase in the odds of developing Acute Kidney Injury in the general population and in people with Chronic Kidney Disease. This study found that the chances of developing ARI increased by more than 50% in people who were exposed to NSAIDs in the general population and in people with CKD, and in older people the chance of developing ARI doubled. Future research must quantify the level of high-dose use in patients with CKD and explore the effects of comorbidities and co-prescription.
O tétano neonatal, pode acometer os neonatos durante os seus primeiros 28 dias de vida. A doença se manifesta poucos dias após o parto com choro, irritabilidade e dificuldade progressiva na amamentação. Segundo estudos, é uma moléstia frequente em populações carentes, que possuem o acesso às unidades de saúde prejudicado. O objetivo deste presente estudo foi analisar os casos de tétano neonatal durante o período de dezembro de 2015 a dezembro de 2020 e suas repercussões nas regiões brasileiras. Estudo de cunho transversal, observacional e descritivo, realizado através de um levantamento de dados presentes no site do DATASUS, analisando o número de internações; média permanência; taxa de mortalidade, no intervalo de tempo de dezembro de 2015 a dezembro de 2020 em relação a morbidade do CID-10 tétano neonatal. Durante o intervalo de tempo de dezembro de 2015 a dezembro de 2020, no Brasil, ocorreram o total de 37 internações devido ao tétano neonatal. No caso da média de dias permanência, a região Nordeste possui o maior valor, com 11,2, seguida da região Sul com 8,0. A respeito da taxa de mortalidade, apenas as regiões Nordeste (16%) e Sudeste (50%) apresentam dados presentes no DATASUS. O tétano neonatal segue como um grande problema de saúde pública; é uma moléstia que acomete populações em carentes condições de vida, onde há dificuldade de acesso às unidades de saúde. É das doenças mais subnotificadas no Brasil. Por ser uma doença de grave prognóstico, demanda internação e cuidados intensivos. O tétano neonatal é uma patologia subnotificada, prejudicando estudos acerca da sua distribuição e morbidade para o país. O nordeste do país sofre com o maior número de internações e média de permanência devido as precárias condições de alguns municípios, todavia a região Sudeste lidera em relação a taxa de mortalidade.
To evaluate the prevalence of smoking in patients who have suffered acute myocardial infarction with ST-segment elevation. Results: The findings of this study suggested that predictors of plaque erosion varied with different smoking status in patients with STEMI and broaden clinicians' understanding of clinical and lesion predictors of plaque erosion, especially in relation to smoking. Conclusion: In current smokers, diabetes mellitus was negatively associated with plaque erosion compared with plaque rupture. In nonsmokers, higher MLA and close bifurcation were positively related to plaque erosion, but not to plaque rupture. Compared to plaque rupture, the correlation between plaque erosion and current smoking complements clinicians' understanding of plaque erosion. There was a strong indication of the relationship between smoking and the development of STEMI.
At the end of 2019, cases of a new acute viral respiratory disease, later recognized as COVID-19, were described. The pandemic has greatly affected healthcare services around the world and is in addition to existing challenges for emergency services such as ST-elevation myocardial infarction. Both conditions can coexist, initial presentations can overlap, and there is no such thing as a true and reliable point-of-care test. Pre-hospital diagnosis and timely treatment of acute coronary syndrome are necessary to achieve optimal outcomes. The use of risk stratification tools (such as GRACE scores) can assist in prioritizing cases to minimize their hospital stay. Critically ill patients with STelevation myocardial infarction, no catheter lab facilities in place or too unstable for transfer must be considered for thrombolysis and activation of services for facilitated PCI, if needed. Patients with cardiovascular disease are particularly vulnerable during this period, regardless of their infectious status. Adapting cardiac services to ensure continuity of care for these patients, even in the context of a new wave of COVID-19, is essential to minimize preventable cardiovascular death.
