Background: This study aimed to evaluate the relationship between quadriceps muscle thickness and functional performance on the 60s sit-to-stand test (60s-STS), the six-minute walk test (6MWT), and handgrip strength in non-dialytic stage 4 and 5 chronic kidney disease (CKD) patients. Methods: This was a cross-sectional study that evaluated 40 CKD patients aged between 30-70 years. Participants were submitted to an assessment that included quadriceps muscle thickness evaluated by a portable ultrasound. Functional performance tests included the 60s-STS, distance walked in the 6MWT, and handgrip strength. Also, body composition evaluated using electrical bioimpedance analysis and physical activity level through the short version of International Physical Activity were measured. Multiple linear regression was used to investigate the relationship between the quadriceps thickness and functional performance. Results: Quadriceps muscle thickness was correlated to 60s-STS (R2 = 43.6%; 95% CI = 0.022 - 0.665; β = 0.34; p = 0.037). Also, a moderate correlation between this muscle thickness and appendicular skeletal muscle (ALM) was found in CKD patients (r = 0.603, p <0.001). No relationship was found between quadriceps muscle thickness with the 6MWT and handgrip strength. Conclusion: Quadriceps muscle thickness is associated to 60s-STS, thus our results demonstrate the repercussions of the disease on the musculoskeletal system.
RESUMO Este estudo avaliou o nível de atividade física de pacientes em tratamento hemodialítico, verificando sua relação com variáveis sociodemográficas e laboratoriais. Foi realizado estudo de corte transversal composto por 108 sujeitos com doença renal crônica sob hemodiálise, sendo constituídos os grupos "ativo" e "sedentário". Foram avaliados dados socioeconômicos (questionário semiestruturado), características da doença renal, nível de atividade física (International Physical Activity Questionnaire - IPAQ) e dados laboratoriais (hematócrito, hemoglobina, creatinina, albumina, ureia) dos últimos exames registrados em prontuários. 8% dos pacientes da amostra foram identificados como sedentários, e 70,4% não receberam orientação para a realização de atividade física (p=0,013). Não foram observadas correlações entre o nível de atividade física e os dados socioeconômicos, clínicos e os parâmetros bioquímicos. Concluiu-se que indivíduos doentes renais crônicos em terapia renal substitutiva do tipo hemodiálise apresentam baixo nível de atividade física. Este achado está relacionado com a frequência de orientações a esse respeito para a população em diálise, não estando relacionado a dados sociodemográficos, clínicos e bioquímicos avaliados.
This study aimed to evaluate the association between the presence of depressive symptoms and physical activity level (PAL) in patients with chronic kidney disease undergoing hemodialysis (HD). Cross-sectional study comprising 101 patients on HD, mean age 47.4±12.5 years. Sociodemographic data, economic, clinical, assessment of depressive symptoms (Beck Depression Scale - BDI), PAL (International Physical Activity Questionnaire - IPAQ), guidance for performing physical activity (PA) and time of HD. The prevalence of sedentary and depressive symptoms was 79.2% and 39.6%, respectively. Depressive symptoms were observed in 47.5% of sedentary patients (χ2=10.029, p=0.002), the ratio of prevalence of these symptoms five times higher in this group. 68.3% of patients on HD received no guidance for PA. Under 49 months of hemodialysis treatment, 63.8% of sedentary (χ2=3.011, p=0.083) and 57.5% of patients with depressive symptoms (χ2=0.100, p=0.752) already had these features. Although little is known about the relationship between depression and PA among patients undergoing HD, this study found that depressive symptoms were more prevalent among inactive. The results of this study demonstrate that patients undergoing HD have low PAL, relative presence of depressive symptoms and high lack of guidance on PA.
ObjectiveTo evaluate the effect of a 12‐week whole‐body vibration (WBV) training program on quadriceps muscle strength, quadriceps thickness, the 6‐min walk test (6MWT), respiratory muscle strength and quality of life in adults with a kidney transplant.MethodFive renal transplant recipients (age: 43–52 years; body mass: 67.1 kg; height: 1.60 m) were examined individually before and after a 12‐week WBV program (two weekly sessions, 35 Hz frequency, 2–4 mm amplitude) using quadriceps strength, quadriceps thickness, 6‐min walking test, inspiratory muscle strength and quality of life.ResultsThere were no complications during WBV protocol, and the program was considered safe. Two of the participants changed quadriceps muscle strength, and one increased muscle thickness. Inspiratory muscle strength improved in one participant. The distance covered on 6MWT increased for two participants. Most of the subjects (P3, P4 and P5) reported improved quality of life, mainly regarding pain and social aspects.ConclusionThe protocol improved the physical function and quality of life in some of the patients. However, WBV training prescription should be adjusted according to individual needs. Thus, studying the use of WBV in this patient group is necessary to understand and apply the benefits for this population.
