Introduction: To address the increasing prevalence of chronic diseases, in an environment with limited healthcare resources, we propose a new realistic approach to provide healthcare services at the patient's home. Objective: To describe the effect of a telemonitoring program for patients with heart failure (HF) and or chronic lung disease (CLD) regarding clinical efficacy, medical services use and quality of life (QoL), at 3-6 months of inclusion.
