Objectives: Both periodontitis and osteoporosis represent major health problems especially in elderly women. The relationship between the two diseases and oral bone loss is important having significant public health impact in the prevention of morbidity and mortality related to these disorders. The present study was aimed to investigate the possible association between osteoporosis and periodontal disease among postmenopausal women residing in Goa, India. Study design: A complete periodontal examination (all teeth except third molar) including plaque index (PI), gingival index (GI), clinical attachment loss (CAL) measurement was performed on 80 dentate Goan postmenopausal women (age ≥ 50 yrs) with generalized chronic periodontitis. Mean alveolar bone loss (ABL) was measured from full mouth intraoral periapical radiographs, by recording the distance from cementoenamel junction (CEJ) and the most coronal portion of alveolar crest at mesial and distal aspect of all teeth except canines and third molars. Systemic bone loss was determined from hand-wrist radiograph of the patient through Digital X-Ray Radiogrammetry. Statistical analysis was done to assess the relationships between periodontal variables and bone mineral density (BMD) after adjusting for age, years since menopause, body mass index (BMI), smoking, number of remaining teeth, PI and GI. Results: Age of the patient, years since menopause and BMI showed significant correlation with BMD. CAL and ABL showed mildly negative and statistically nonsignificant correlation with the BMD. Of all the variables studied, only smoking and BMI were strong predictors of BMD. Conclusion: Skeletal BMD is related to interproximal ABL and CAL, though not to a statistically significant level; implicating postmenopausal osteopenia as a risk indicator for periodontal disease.
A 34 year old woman reported for evaluation of generalized gingival swelling pain and bleeding. The patient also reported menstrual irregularities. Based on the oral and systemic examinations and hematological investigations, a provisional diagnosis of ‘menstrual cycle associated gingivitis’ was made. The patient was referred for a gynecologic consultation. At the three-week dental recall, a worsening of gingival overgrowth with a necrotic component was noted. The hematologic investigations revealed markedly elevated leukocyte counts. The clinical and hematological findings led us to a diagnosis of leukemia. This report emphasizes the importance of the dentist as well as the physician (in this case, the gynecologist) in correlating the oral, systemic and hematological findings in the diagnosis and also of regular recall in the management of a case.
The present randomized, double-blinded, placebo-controlled study investigated the effect of a standardized fenugreek extract (FHE) on perimenopausal discomforts and its influence on hormonal balance and safety. Healthy women characterized with perimenopausal symptoms (n = 48), as assessed by MRS questionnaire, were randomized either to FHE (n = 24) or placebo (n = 24) and supplemented with 250 mg × 2/day for 42 days. Both inter and intra-group comparison revealed a significant improvement in somatic, psychological, and urogenital scores in FHE group, especially for hot flashes (25.9%), night sweats (26.5%), depression (31.8%), and insomnia (21.6%). Further hormone analysis revealed an enhancement in serum estradiol (18.9%), free testosterone (38.2%), and progesterone (19.9%) concentrations and a significant decrease in FSH (38.2%) and SHBG (21.1%) concentrations toward establishing a hormonal balance among FHE-group; without significant changes in other clinical safety parameters. Thus, FHE supplementation offered a significant reduction in vasomotor effects and depression in perimenopausal women, without any adverse effects Practical applications Fenugreek is a popular kitchen spice and Ayurvedic medicine for a variety of health conditions including diabetes, hypercholesterolemia, hepatotoxicity, gastritis, and also for a variety of hormone-related health conditions such as sexual functions, lactation, osteoporosis, PCOS, and post/perimenopausal discomforts. Fenugreek is rich in alkaloids, steroidal saponins, flavonoids and 4-hydroxyisoleucine. The present randomized-controlled study investigated the plausible application of a standardized hydro-ethanolic extract of fenugreek seeds (FHE) having a unique 3:1 ratio for protodioscin to trigonelline in the management of perimenopausal discomforts. It was observed that FHE at a dosage of 250 mg × 2/day for 42 days significantly reduced the discomforts, especially vasomotor symptoms and depression, and helped to attain a hormonal balance without any adverse effects or deviations in clinical safety How to cite this article: Khanna A, John F, Das S, et al.
