ABSTRACT. Five out of ten patients with acute or subacute farmer's lung disease had granuloma‐like lesions in splenic fine‐needle aspirates. The splenic lesions were indistinguishable from certain lesions usually seen in early sarcoidosis. It is suggested that the immunological response in extrinsic allergic alveolitis in an acute stage is not exclusively defined to the pulmonary tissue.
