Objective To determine the effectiveness of a sanitation campaign that combines "shaming" (i.e. emotional motivators) with subsidies for poor households in rural Orissa, an Indian state with a disproportionately high share of India's child mortality. Methods Using a cluster-randomized design, we selected 20 treatment and 20 control villages in the coastal district of Bhadrak, rural Orissa, for a total sample of 1050 households. We collected sanitation and health data before and after a community-led sanitation project, and we used a difference-in-difference estimator to determine the extent to which the campaign influenced the number of households building and using a latrine. Findings Latrine ownership did not increase in control villages, but in treatment villages it rose from 6% to 32% in the overall sample, from 5% to 36% in households below the poverty line (eligible for a government subsidy) and from 7% to 26% in households above the poverty line (not eligible for a government subsidy). Conclusion Subsidies can overcome serious budget constraints but are not necessary to spur action, for shaming can be very effective by harnessing the power of social pressure and peer monitoring. Approximately 3 billion people, most of whom live in Asia, Africa, and the Americas, rely on solid fuels (i.e. wood, crop wastes, dung, charcoal) and kerosene for their cooking needs. Exposure to household air pollution from burning these fuels is estimated to account for approximately 3 million premature deaths a year. Cleaner fuels – such as liquefied petroleum gas, biogas, electricity, and certain compressed biomass fuels – have the potential to alleviate much of this significant health burden. A wide variety of clean cooking intervention programs are being implemented around the world, but very few of these efforts have been analyzed to enable global learning. The Clean Cooking Implementation Science Network (ISN), supported by the U.S. National Institutes of Health (NIH) and partners, identified the need to augment the publicly available literature concerning what has worked well and in what context. The ISN has supported the development of a systematic set of case studies, contained in this Special Issue, examining clean cooking program rollouts in a variety of low- and middle-income settings around the world. We used the RE-AIM (reach, effectiveness, adaptation, implementation, maintenance) framework to coordinate and evaluate the case studies. This paper describes the clean cooking case studies project, introduces the individual studies contained herein, and proposes a general conceptual model to support future planning and evaluation of household energy programs.
Household cooking using solid biomass fuels is a major global health and environmental concern. As part of the Research on Emissions Air quality Climate and Cooking Technologies in Northern Ghana study, we conducted 75 in-field uncontrolled cooking tests designed to assess emissions and efficiency of the Gyapa woodstove, Philips HD4012, threestone fire and coalpot (local charcoal stove). Emission factors (EFs) were calculated for carbon monoxide (CO), carbon dioxide (CO), and particulate matter (PM). Moreover, modified combustion (MCE), heat transfer (HTE) and overall thermal efficiencies (OTE) were calculated across a variety of fuel, stove and meal type combinations. Mixed effect models suggest that compared to traditional stove/fuel combinations, the Philips burning wood or charcoal showed significant fuel and energy based EF differences for CO, but no significant PM changes with wood fuel. MCEs were significantly higher for Philips wood and charcoal-burning stoves compared to the threestone fire and coalpot. The Gyapa emitted significantly higher ratios of elemental to organic carbon. Fuel moisture, firepower and MCE fluctuation effects on stove performance were investigated with mixed findings. Results show agreement with other in-field findings and discrepancies with some lab-based findings, with important implications for estimated health and air quality impacts.
In Spring/Summer 2020, most individuals living in the United States experienced several months of social distancing and stay-at-home orders because of the coronavirus (COVID-19) pandemic. Clinicians, restaurant cooks, cashiers, transit operators, and other essential workers (EWs), however, continued to work outside the home during this time in order to keep others alive and maintain a functioning society. In the United States, EWs are often low-income persons of color who are more likely to face socioeconomic vulnerabilities, systemic racism, and health inequities. To assess the various impacts of COVID-19 on EWs, an online survey was distributed to a representative sample of individuals residing in six states during May/June 2020. The sample included 990 individuals who identified as EWs and 736 nonessential workers (NWs). We assessed differences between EW and NW respondents according to three categories related to health equity and social determinants of health: (1) demographics (e.g. race/ethnicity); (2) COVID-19 exposure risk pathways (e.g. ability to social distance); and (3) COVID-19 risk perceptions (e.g. perceived risk of contracting COVID-19). EWs were more likely to be Black or Hispanic than NWs and also had lower incomes and education levels on average. Unsurprisingly, EWs were substantially more likely to report working outside the home and less likely to report social distancing and wearing masks indoors as compared to NWs. EWs also perceived a slightly greater risk of contracting COVID-19. These findings, which we discuss in the context of persistent structural inequalities, systemic racism, and health inequities within the United States, highlight ways in which COVID-19 exacerbates existing socioeconomic vulnerabilities faced by EWs.
This study uses data from a survey of coastal Miami-Dade County, Florida, residents to explore how different types of forecast and warning messages influence evacuation decisions, in conjunction with other factors. The survey presented different members of the public with different test messages about the same hypothetical hurricane approaching Miami. Participants’ responses to the information were evaluated using questions about their likelihood of evacuating and their perceptions of the information and the information source. Recipients of the test message about storm surge height and the message about extreme impacts from storm surge had higher evacuation intentions, compared to nonrecipients. However, recipients of the extreme-impacts message also rated the information as more overblown and the information source as less reliable. The probabilistic message about landfall location interacted with the other textual messages in unexpected ways, reducing the other messages’ effects on evacuation intentions. These results illustrate the importance of considering trade-offs, unintended effects, and information interactions when deciding how to convey weather information. Recipients of the test message that described the effectiveness of evacuation had lower perceptions that the information was overblown, suggesting the potential value of efficacy messaging. In addition, respondents with stronger individualist worldviews rated the information as significantly more overblown and had significantly lower evacuation intentions. This illustrates the importance of understanding how and why responses to weather messages vary across subpopulations. Overall, the analysis demonstrates the potential value of systematically investigating how different people respond to different types of weather risk messages.
