Exposure-response analyses were performed to test the microbiological and clinical efficacies of tigecycline in complicated intra-abdominal infections where Escherichia coli and Bacteroides fragilis are the predominant pathogens. Data from evaluable patients enrolled in three clinical trials were pooled. Patients received intravenous tigecycline (100-mg loading dose followed by 50 mg every 12 h or 50-mg loading dose followed by 25 mg every 12 h). At the test-of-cure visit, microbiological and clinical responses were evaluated. Patients were prospectively classified into cohorts based on infection with a baseline pathogen(s): E. coli only (cohort 1), other mono-or polymicrobial Enterobacteriaceae (cohort 2), at least one Enterobacteriaceae pathogen plus an anaerobe(s) (cohort 3), at least one Enterobacteriaceae pathogen plus a gram-positive pathogen(s) (cohort 4), and all other pathogens (cohort 5). The cohorts were prospectively combined to increase sample size. Logistic regression was used to evaluate ratio of steady-state 24-hour area under the concentration-time curve (AUC) to MIC as a response predictor, and classification-and-regression-tree (CART) analyses were utilized to determine AUC/MIC breakpoints. Analysis began with cohorts 1, 2, and 3 pooled, which included 71 patients, with 106 pathogens. The small sample size precluded evaluation of cohorts 1 (34 patients, 35 E. coli pathogens) and 2 (16 patients, 24 Enterobacteriaceae). CART analyses identified a significant AUC/MIC breakpoint of 6.96 for microbiological and clinical responses (P values of 0.0004 and 0.399, respectively). The continuous AUC/ MIC ratio was also borderline predictive of microbiological response (P ؍ 0.0568). Cohort 4 (21 patients, 50 pathogens) was evaluated separately; however, an exposure-response relationship was not detected; cohort 5 (31 patients, 60 pathogens) was not evaluated. The prospective approach of creating homogenous populations of pathogens was critical for identifying exposure-response relationships in complicated intra-abdominal infections.Evaluating exposure-response relationships by use of clinical trial data is an essential component of optimizing antimicrobial treatment, yet it is often quite challenging. A single dosing regimen is often used, thus limiting the range of observed drug exposure, and it is difficult to collect on an individual-patient basis the three integral pieces of information required to perform such analyses: pharmacokinetic (PK), clinical, and microbiological outcome data. BackgroundOMC, an aminomethylcycline structurally related to tetracycline agents, was recently approved by the US FDA for the treatment of adult patients with ABSSSI (IV-to-PO and PO regimens) and CABP (IV-to-PO regimen). To better understand exposure-related concerns for ALT increase, PK-PD relationships for ALT were evaluated using data from OMC-treated patients enrolled in three Phase 3 studies.MethodsRepeated-measures multiple linear regression was used to evaluate factors predictive of ALT, including different OMC total-drug AUC measures prior to each ALT, with interactions and covariates selected stepwise. Using a final AIC-optimized model, predicted percent probabilities of ALT elevation >1, 1.5, 2, 3, 5, and 10 × upper limit of normal (ULN) at any time post-baseline and up to 2 days after the end of therapy were calculated among analysis patients for fixed post-baseline OMC AUC measures, and among simulated patients after IV-to-PO and PO dosing regimens.ResultsThe final model, developed using data from 327 patients with PK, included increased prior cumulative AUC among males, increased baseline gamma-glutamyl transferase, and study (ABSSSI, PO only) as factors predictive of increased ALT (P < 0.0001). However, the model-predicted impact of OMC across fixed average daily AUC values on ALT elevation endpoints of >1, 1.5, 2, 3, 5, and 10 × ULN was minimal (Figure 1), even for males despite the interaction with AUC (Figure 2). Among all patients, the estimated increases in percent probabilities for the 90th percentile of AUC relative to zero AUC were 5.81, 5.20, 1.53, 0.61, 0.31, and 0%, respectively, and ≤11.5, 7.76, 8.33, 1.31, 1.31, and 0%, respectively, across single variable patient subsets. Percent probabilities of ALT elevation endpoints were within 2.84% when comparing simulated and observed patients after administration of OMC IV-to-PO and PO dosing regimens (Figure 3).ConclusionA statistically significant relationship between an increase in ALT and increase in OMC AUC for males in the presence of other factors was found. However, increases in model-predicted ALT elevation endpoints across fixed OMC AUC values, or among simulated patients after administration of OMC IV-to-PO and PO dosing regimens relative to observed patients, were minimal. Background SUL-DUR is a β-lactam/β-lactamase inhibitor combination in development for the treatment of ABC infections, which are often severe and associated with substantial mortality. PPK, PTA, and clinical PK/PD analyses were conducted using all available PK and efficacy data to support SUL-DUR dose selection. Methods PPK analyses were performed using PK data from 373 subjects, including 110 patients who received SUL-DUR and underwent PK sampling in the pivotal Phase 3 (ATTACK) trial. SUL-DUR concentrations in epithelial lining fluid (ELF) from healthy subjects were utilized to predict SUL-DUR ELF concentrations in Phase 3 patients. Covariate analyses evaluated the impact of demographic factors on PK exposures. The PPK model, non-clinical PK/PD data, and in vitro surveillance data were used to assess the PTA by MIC in simulated patients with ABC infections. Clinical PK/PD relationships were assessed using dichotomous and time-to-event efficacy endpoints from the ATTACK trial. Results Two-compartment models with linear kinetics best characterized SUL and DUR PK. Consistent with high renal clearance of SUL-DUR, dose adjustments are needed in patients with renal impairment or augmented renal function. Body weight, site of infection, and East Asian region (which grouped patients from mainland China, Taiwan, and South Korea vs others) were statistically significant covariates in the PPK analysis but were not clinically relevant. No other statistical or clinically relevant covariates were identified. As shown in Figures A and B for plasma and ELF, respectively, the PTA was ≥ 90% for pathogens with an MIC ≤ 4 µg/mL across renal function categories in simulated patients, using PK/PD targets associated with a 1-log kill10. No relationships were observed between PK/PD indices and efficacy endpoints, consistent with most patients achieving SUL and DUR exposures above nonclinical PK/PD targets for efficacy.
