Background. To investigate the frequency of restless leg syndrome (RLS), sleep quality impairment, depression, fatigue, and sleep behavior disorder and to determine the effects of surgery on these parameters in radiculopathy patients resistant to conservative treatment. Methods. The present study included 66 lumbar radiculopathy patients, who were resistant to conservative treatment and had indication of surgery. Five different questionnaires were performed to assess depression (the Beck Depression Inventory (BDI)), sleep quality (the Pittsburgh Sleep Quality Index (PSQI)), fatigue (the Fatigue Severity Scale (FSS)), and presence of RLS and rapid eye movement sleep behavior disorder (RBD). The same questionnaires were also performed on a control group (n = 61). Results. Of the radiculopathy patients, 68.1% had RLS and 92.4% had fatigue. Of the controls, 16.4% had RLS and 59% had fatigue. RBD was present in 8 (12.1%) patients and 3 (4.9%) controls. The PSQI revealed that sleep quality was impaired in 46 (69.7%) patients and 35 (57.4%) controls (P > 0.05). The number of individuals having substantial depression according to the BDI was significantly higher in the patients than in the controls. Conclusions. There was a significant increase in the frequency of RLS, which was significantly decreased in the postoperative period in the radiculopathy patients.
Amaç: Pulse radyofrekans (PRF) tedavisi lomber faset sendromuna (LFS) bağlı ağrı tedavisi için analjezi sağlamada alternatif tedavi olarak önerilmektedir. Bu çalışmada klinik başarının yanında sağlıkla ilgili yaşam kalitesi üzerine etkinliğinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: LFS nedeni ile PRF uygulaması yapılan hastalarda klinik etkinliği değerlendirmek için Vizüel Ağrı Skalası (VAS) ve Oswestry Özürlülük İndeksi (OÖİ) kullanıldı. Ankylosing spondylitis is a chronic inflammatory disease. It might affect the facet, sacroiliac joint, and vertebra regions. It leads to thoracolumbar kyphotic deformity in the spine region. Both ankylosing spondylitis and kyphotic deformity impair the patients' quality of life. Our objective was to assess the impact of the correction of kyphotic deformity in ankylosing spondylitis on health-related quality of life. 11 thoracolumbar kyphosis patients diagnosed with ankylosing spondylitis were operated and their deformity was corrected. Medical Outcome Study Short Form-36 (SF-36), visual analog scale (VAS), and Oswestry disability index (ODI) studies were performed on patients before surgery and 1 month after surgery to assess their impact on health-related quality of life. In our study, postoperative changes in the sub-parameters of the SF-36 test, which was used to assess the health-related quality of life of patients, were determined to be statistically significant compared to post-surgery. Likewise, postoperative changes in VAS and ODI tests compared to pre-surgery were also statistically significant. Corrective surgery of thoracolumbar kyphosis in patients with ankylosing spondylitis has a positive impact on both better health-related quality of life and recovery of back and low back pain.
Rijit kifoskolyoz hastalığının tedavisinde çözülmesi gereken birçok problem bulunur. Bu çalışmada amaç sorunların tedavisinde kullanılan halo gravite traksiyonu uygulanmasının hastalarda günlük yaşam aktivitelerine, fonksiyonlarına ve ruhsal durumlarına etkilerini ortaya koymaktı. Gereç ve Yöntemler: Rijit kifoskolyoz tanısı ile cerrahisi yapılacak hastalara cerrahi öncesi halo gravite traksiyonu uygulandı. Çalışmamıza traksiyon tedavisi gören toplam 14 hasta alındı. Toplam 14 hastanın %28,6'sı (n=4) erkek, %71,4'ü kadın (n=10) idi. Ortalama yaş 17.22 ± 6.15 idi. Hastaların 6'sı (%42,8) idiyopatik skolyoz, 5'i (%35,7) konjenital skolyoz ve 3'ü (%21,5) nöromüsküler skolyozdu. Hastalara işlem sonrası 10'uncu gün ve traksiyon işlemi sonrası birinci ayda yaşam kalite ölçeklerinden Medical Outcome Study Short Form-36 (SF-36) ile değerlendirilmesi yapıldı. Bulgular: Halo traksiyon uygulamasının verilerinin analizi için Wilcoxon Signed Ranks Testi kullanıldı. İstatistiksel anlamlılık düzeyi p <0.05 olarak belirlendi. Verilerin istatistik analiz sonucu fiziksel fonksiyon, sosyal işlev, fiziksel rol güçlüğü, emosyonel rol güçlüğü, ruhsal sağlık, enerji/canlılık/vitalite ve ağrı parametrelerindeki değişiklikler p <0.05 olup anlamlıydı. Genel sağlık algısındaki değişiklikler ise istatistiksel anlamlı değildi. Sonuç: Rijit kifoskolyoz olan hastalarda uygulanan halo gravite traksiyon yönteminin etkinliğinin yanında hastanın traksiyon süresi boyunca yaşam kalitesi üzerindeki olumlu etkisinin traksiyona adaptasyon süresi ile ilişkilidir. Günlük traksiyon eğitiminin ve bakımının bu adaptasyonda önemli olmaktadır. Hastaların uyku düzeni ve ağrı sorunlarının çözülmesi traksiyon süresince yaşam kalitesi üzerine pozitif yönde etkilidir.
Objective: It is well-established that vertebroplasty has effects on sagittal parameters due to various pathologies of the spine. Our objective in the study was to reveal the impact of vertebroplasty that was applied to compression fractures due to trauma on sagittal parameters. Material and Method: Twenty patients were included in the study. Scoliosis radiographs were shot preoperatively and at first month during the postoperative period. Spinopelvic parameters of each patient including pelvic incidence (PI), sacral slope (SS), pelvic tilt (PT), lumbar lordosis (LL), thoracic kyphosis (TK), thoracolumbar kyphosis (TLK), cervical lordosis (CL), and sagittal vertical axis (SVA) were assessed. Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI) assessments of the patients were performed as well. Results: Spinopelvic parameter values of the patients were assessed statistically through the analysis of the Paired sample T test before surgery and in the first month during the postoperative period. It was determined that the changes in the PT, SS and LL values of the patients between preoperative and first month postoperative were at the level of p 0.659, p=0.716, p=0.012; and the changes were not significant. It was determined that the changes in the TL, TK, CL, SVA, VAS and ODI values of the patients between preoperative and first month postoperative were at the level of p <0.001and the changes were significant. Conclusion: Vertebroplasty is an efficient technique for relieving pain in thoracic fractures that have a score of 4 and over, based on the thoracolumbar injury classification score and that are type A, based on the AO Spine thoracolumbar injury classification score. Elimination of pain in the acute period is effective in ameliorating the sagittal parameters of patients.
