Computer-based cognitive retraining (CBCR) intervention has gained great popularity in recent years. This study aimed to investigate the occurrence of skill generalization to daily living task for individuals with acquired brain injury (ABI) after completion of eight modules of a commercially available CBCR program, the Parrot Software. The study investigated changes in individuals' global cognition as measured by the Montreal Cognitive Assessment, and changes in individuals' performance during a medication-box sorting task, a novel instrumental activity of daily living. The medication-box sorting task resembled real life medication management with daily prescribed and over-the-counter medications. Twelve individuals with ABI from a community-based program completed the study. Results indicated that CBCR intervention brought about improvement in global cognition, but the improvement did not appear in any particular cognitive domain. Additionally, the gains in global cognition failed to enhance performance in the medication-box sorting task. This exploratory study demonstrated that while CBCR may be a promising intervention for improving global cognition in individuals with ABI, additional intervention might be needed for generalization to occur to a novel daily task. Future studies should look for the ultimate therapeutic outcome from CBCR interventions or include interventions that could bridge the gap between CBCR intervention and performance improvement in daily living occupations.
This study explored the effectiveness of a modified Lifestyle-integrated Functional Exercise program for increasing community-dwelling older adults' lower body strength and balance to decrease fall risk. Methods: Purposive sampling of men and women aged 65 years and older, with or without a history of falls, living at retirement communities yielded 19 older adult participants, and 16 of the participants completed the 26-week integrated exercise program. The program consisted of five-group training sessions focused on how to integrate individualized exercises into everyday activities, followed by 20 weeks of independent practice with a booster session at Week 10 and two phone calls at Week 15 and Week 20. A battery of assessments was used 3 times to measure the participants. Results: Results demonstrated a significant improvement in lower body strength and balance, but fall risk reduction cannot be confirmed from this study. Conclusion: Despite reduction in fall risk was inconclusive from this study, a modified Lifestyle-integrated Functional Exercise program delivered to community-dwelling older adults in a group format may be an effective intervention program to improve lower body strength and balance, while integration of exercises into daily activities may also appear to be more sustainable than traditional exercise program.
The goal for pre-admission interview is to identify students who will be successful both academically and professionally. The traditional structured pre-admission interview has not been shown to be effective in predicting students' success in academic and fieldwork performance. This article describes an innovative behavioral interview process during which applicants interviewed simulated clients as part of the preadmission selection process. The goals of the pre-admission behavioral interview were to observe applicants' behavior as team players, and to assess their interpersonal communication skills, capability to be reflective, and professionalism. During the behavioral interview, faculty interviewers assessed the applicants' performance in the planning phase, the simulated client interview phase and the reflection phase. The behavioral interview was evaluated with a feasibility study using a mixed methods, single group exploratory design. Results showed that applicants, simulated clients, and faculty interviewers positively endorsed the behavioral interview. In addition, applicants found the behavioral interview less intimidating than a traditional structured interview, and appreciated the opportunity to interact with faculty and demonstrate their communication skills and ability to collaborate in a team. Similarly, faculty interviewers found the behavioral interview fruitful when observing the applicants "in action" and displaying their non-academic attributes, demonstrating potential to be successful students in the program. While being one of the stronger predictors, the behavioral interview scores only weakly correlated with the admitted applicants' first semester Grade Point Averages. Further longitudinal evaluation may demonstrate the potential of the behavioral interview as a pre-admission tool identifying students who may be successful both in academic and fieldwork performance.
Introduction: In spite of the vast amount of literature on pet therapy and dog companionship, limited studies exist on older adults with vision loss and the experience of owning a dog guide. The purpose of this study is to explore the facilitators and barriers of first-time ownership and utilization of a dog guide as experienced by older adults with vision loss. Methods: Data were gathered among seven participants using open-ended semistructured telephone interviews. Participants described their experiences related to owning a dog guide. Using phenomenological analysis, themes were extracted from verbatim transcriptions. Results: Through constant comparison methods, five themes emerged: increased responsibilities for new dog guide owners, changes in habits and routines, quick human–dog guide bonding, increase in community integration, and enhancement of autonomy through dog guide ownership. Discussion: The study results suggest that obtaining a dog guide increased the older adults with vision loss everyday engagement in community activities. The increased confidence in independent mobility may have led participants to engage in activities in unfamiliar environments, thus improving their autonomy, self-esteem, and physical abilities. These changes resulted in increased feelings of independence and freedom for the older adults with vision loss. Participants also revealed positive changes in their daily habits. Due to the increased physical ability and motivation needed to complete activities, making adjustments to owning a dog guide became easier. Furthermore, a human–dog guide bond was prevalent among all seven participants. Implications for practitioners: Themes extracted provide health practitioners and dog guide organizations insight into how owning dog guides may empower older adults with vision loss.
