Side 4Forord 2018 DHR's styregruppe praesenterer hermed årsrapporten for 2018.Først og fremmest tak til alle laeger, sekretaerer og andet personale, der yder et stort stykke arbejde for indrapportering til DHR. Også en stor tak til vores sekretariat, der sørger for den gode kontakt til afdelingerne. Endelig tak til styregruppen for det solide arbejde der altid udføres! Rapporten er desvaerre blevet forsinket endnu en gang på, hvilket skyldes aendringer i databasens programmeringsplatform. Den nye platform skulle gerne fremadrettet gøre det lettere at danne rapporten. Vi har nu brug for stabilitet i RKKP, således at vi kan få mulighed for at udvikle indikatorerne samt nye analyser. AEndringer i styregruppenOverlaege ph.d. Kirill Gromov, Hvidovre Hospital, er udpeget som ny repraesentant Region Hovedstadens i DHR. Jeg vil gerne takke tidligere overlaege Jens Retpen, Region Hovedstaden, for det store arbejde han har gjort i hele DHR's levetid. Hans gode humør, engagement, og skarpe blik har beriget styregruppen. Vi er taknemmelige for det store arbejde som Jens har gjort. Ny epidemiolog i styregruppen er Epidemiolog Pernille Iversen, Regionernes Kliniske Kvalitetsudviklings Program. Vi takker Helle Collatz Christensen for hendes entusiasme i de seneste par år. Velkommen til de nye. Årets rapportDHR lever op til gaeldende krav for indrapportering i forhold til indikatorerne. Dette års komplethedsgrad på 98 % for primaer THA er meget tilfredsstillende, mens kompletheden for revisions THA er på 92 %. Såfremt der fratraekkes revision af hemialloplastik indberettet til LPR som revisions THA, stiger den til 95 %. Derfor opfyldes standarden for både primaere og revisioner. Der er i perioden 1995-2017 i alt indberettet ca. 161.968 primaere og 25.208 revisions THA'er til DHR. Der har vaeret en svag stigende tendens i antallet af udførte alloplastikker de sidste to år, hvilket blandt andet kan forklares ud fra en større aeldre befolkning. En eventuel incidens stigning i den yngre befolkning er ikke undersøgt, men der sker sandsynligvis også en stigning her. Indikatorerne; rapportens omdrejningspunktNedlaegning af transfusionsdatabasen har desvaerre betydet at transfusionsdata ikke kan praesenteres, hvorfor indikator 2 ikke kan fremlaegges i denne rapport. Vi håber, at RKKP og danske regioner får styr på dette til naeste år. Genindlaeggelse indenfor 30 dage efter primaer THA har vist sig at variere meget fra region til region og ikke mindst imellem afdelinger. F.eks. har Region Syddanmark ca. 5 % genindlaeggelse efter operation for artrose, mens Region Hovedstaden og Sjaelland har ca. det dobbelte.Med henblik på at forbedre patientbehandlingen, anbefales det at lave audit ikke bare på denne indikator, men også på pt. der reopereres indenfor 2 år. Introduction Due to inconsistent results and high failure rates, total knee arthroplasty (TKA) is more often used to treat isolated patellofemoral osteoarthritis (PFOA) despite the theoretical advantage of patellofemoral arthroplasty (PFA). It is perceived that second‐generation PFA may have improved the outcomes of surgery. In this systematic review, the primary aim was to compare outcomes of second‐generation PFA and TKA by assessment of patient‐reported outcome measures (PROMs). Methods A systematic search was made in PubMed, Medline, Embase, Cinahl, Web of Science, Cochrane Library and MeSH to identify studies using second‐generation PFA implants or TKA for treatment of PFOA. Only studies using The American Knee Society (AKSS), The Oxford Knee Score (OKS) or The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) to report on PROMs were included. Results The postoperative weighted mean AKSS knee scores were 88.6 in the second‐generation PFA group and 91.8 in the TKA group. The postoperative weighted mean AKSS function score was 79.5 in the second‐generation PFA group and 86.4 in the TKA group. There was no significant difference in the mean AKSS knee or function scores between the second‐generation PFA group and the TKA group. The postoperative weighted mean OKS score was 36.7 and the postoperative weighted mean WOMAC score was 24.4. The revision rate was higher in the second‐generation PFA group (113 revisions [8.4%]) than in the TKA group (3 revisions [1.3%]). Progression of OA was most commonly noted as the reason for revision of PFA, and it was noted in 60 cases [53.1%]; this was followed by pain in 33 cases [29.2%]. Conclusion Excellent postoperative weighted mean AKSS knee scores were found in both the second‐generation PFA group and in the TKA group, suggesting that both surgical options can result in a satisfying patient‐reported outcome. Higher revision rates in the second‐generation PFA studies may in part be due to challenges related to patient selection. Based on evaluation of PROMs, the use of second‐generation PFA seems to be an equal option to TKA for treatment of isolated PFOA in appropriately selected patients. Hopefully, this can be considered by physicians in their daily clinical work. Level of evidence IV.
