Introducción: el recién nacido prematuro de muy bajo peso (RNMBP) es inmunológicamente inmaduro y además presenta una alteración de las barreras naturales de defensa. Objetivo: evaluar los efectos que pueda tener la administración de calostro orofaríngeo, administrado durante los primeros 15 días posnatales, sobre los niveles de inmunoglobulina A (IgA) sérica en recién nacidos prematuros de muy bajo peso durante el primer mes de vida. Material y métodos: se desarrolló un estudio de intervención no aleatorizado con grupo control, en el que se incluyeron 38 recién nacidos con ≤ 32 + 6 semanas de gestación y/o menores de 1.500 g de peso. Los sujetos recibieron 0,2 ml de calostro de su madre cada 4 h, iniciándose el procedimiento en las primeras 24 h de vida hasta el 15.o día postnatal. Se midieron los niveles de IgA en la sangre al nacimiento, 3. er , 15.o y 30.o días de vida. Se registraron datos perinatales al nacimiento y durante el periodo de seguimiento. Resultados: IgA sérica aumentó de forma estadísticamente significativa en el grupo de intervención (M1 15,84 μg/ml, M2 20,07 μg/ml, M3 23,65 μg/ml, M4 30,34 μg/ml, p 0,001) y en el grupo control (M1 12,48 μg/ml, M2 16,48 μg/ml, p 0,018; M3 19,41 μg/ml, M4 22,48 μg/ml, p 0,001). Al mes de vida, los niveles de IgA sérica fueron significativamente mayores en el grupo de intervención que en el grupo control (p 0,026). Introduction: the pandemic caused by SARS-CoV-2 led to the declaration of the state of sanitary alarm between March and June 2020 in Spain. The activity of human milk banks was affected during that period, making it necessary to implement new measures in order to promote milk donation and diminish said impact.Method and objective: the aim of the study was to evaluate the impact of the state of alarm decreed from March 14 to June 22, 2020 on the breastmilk bank at Hospital Virgen de las Nieves, Granada, Spain, in comparison with the same period during the previous year.

Methods Staff members from theses centres were trained, visiting the Milk Bank and with written protocol. A transfer system was stablished with refrigerators and temperature records along the process. The hospital milk bank is responsable for ensuring the traceability of the process. Several variables were analysed and compared: acidity, microbiological culture and volume of pasteurised milk rejected during 2013. Results Two points of attention for milk donors have been operating regularly since 2012. Total number of donors was 43 and 158 litres of donor milk were collected. Analysing the percentage of rejected milk according to its origin:4% the volume received in the bank was not processed, 12% point 2 and 8% point 1 were discarded. This difference lead us to the revision of the storage system, transportation and adequate information for hygiene during milk collection. REFERENCESResults in Table 1 and 2. Background To minimise extrauterine growth restriction in ELGANs computer assisted prescription of parenteral nutrition (CAPPN) was introduced. Aim To evaluate the effectiveness of CAPPN in growth improvement of ELGANs. Patients and methods In this retrospective, observational designed study with a nonprobability, convenience sampling to obtain medical records, we compared 20 ELGANs in the study group after CAPPN to 20 ELGANs in the control group before CAPPN. Daily parenteral and enteral intake of macronutrients, calcium and phosphate in five sequetial time intervals of the first 28 days of life was calculated (day 1-3, 4-7, 2nd, 3rd, 4th week). Outcome measures were the lenght of PN, days to regain birth weight (BW), growth velocity, and weight and head circumpherence (HC) <10th percentile on day 28. Numerical data were analysed by independent-samples t-test or by Mann-Whitney U test, categorical data were analysed by chi-square. Results The combined enteral and parenteral intake of the study group in all five sequential intervals after birth exceeded the intake of the control group. The lenght of PN and days to regain BW did not differ, however growth velocity (14,5 [3,7] vs 11.6 [0,4] g/day (p = 0.03)) and HG velocity (0.9 [0.3] vs 0.7 [0.4] cm/day (p = 0.03)) were higher in the study group. Conclusion CRP has good specificity (96% at 1 mg/L) for CA in preterm infants. Higher initial CRP levels in infants correlate with severity of histological CA.
