Background To improve detection of mucosal lesions during colonoscopy a number of imaging modalities have been suggested, including high definition and virtual chromoendoscopy. Given the theoretical advantage of these new imaging techniques, we aimed to investigate their use for the detection of polyps in patients referred for colonoscopy in a large tertiary hospital. Methods Demographic, endoscopic, and histological data from 1855 consecutive patients undergoing colonoscopy were collected prospectively. Patients were randomly assigned to three endoscopy systems (Fujinon, Olympus, or Pentax) in combination with four modalities: conventional white-light colonoscopy (n = 505), high definition white-light colonoscopy (n = 582), virtual chromoendoscopy (n = 285) and high definition virtual chromoendoscopy (n = 483). Results The mean adenoma detection rate (ADR) was 34.9 %, and the adenoma per colonoscopy rate (APCR) was 2.1. No significant differences were noted between the three endoscopy systems. Moreover, no differences in ADR or APCR were observed between the four imaging modalities. High definition white-light colonoscopy resulted in a significantly higher detection of sessile serrated adenomas (8.2 % vs. 3.8 %; P < 0.01) and adenocarcinomas (2.6 % vs. 0.5 %; P < 0.05) compared with the conventional procedure. Conclusions No significant differences in ADR or APCR between different endoscopy systems, high definition, and/or virtual chromoendoscopy could be observed in routine colonoscopies in the general population. High definition endoscopy was associated with a significantly higher detection rate of serrated adenomas and adenocarcinomas.
Secondary amyloidosis may complicate chronic inflammatory conditions and mostly presents as a renal disease with nephrotic syndrome or renal insufficiency. Its prognosis is largely affected by control of the underlying disease. We report a patient with primary sclerosing cholangitis, who developed cirrhosis over a 4-year period. Therapy with steroids and azathioprine was necessary for symptom control. Despite this treatment, she developed secondary amyloidosis with nephrotic syndrome 4 years after the initial presentation. The inflammatory process in the bile ducts was considered the cause of amyloid A amyloidosis. To control the nephrotic syndrome, liver transplantation was performed with the removal of the diseased liver and bile duct system. Liver transplantation was followed by a progressive and complete disappearance of the nephrotic syndrome. This is the first report describing the occurrence of amyloid A amyloidosis in primary sclerosing cholangitis, and the reversal of a secondary amyloidosis-induced nephrotic syndrome as a result of liver transplantation.
A 76-year-old man was referred to the hospital because of stomach pain, vomiting, and fever persisting for a few days. On physical examination, there was no abdominal tenderness.Initial blood tests revealed normal white cell count and elevated liver aminotransferases (aspartate amino-, and gamma-glutamyl transferase (1328 U/L, normal range <35). Bilirubin was normal. At the emergency unit, computed tomography (CT) was performed showing an infiltrating mass with small rather linear calcifications in the right liver lobe extending through the main bile duct into the pancreatic head. (Fig. 1) Magnetic resonance imaging (MRI) demonstrated a T 2 hyperintense to intermediate intense, T 1 hypointense, diffusion restrictive, complex solid neoplasm with a tubular aspect and slight contrast uptake, extending from the main bile duct into the right intrahepatic bile ducts. There is focal invasion into the cystic duct and the gallbladder. (Fig. 2) The differential diagnosis includes biliary papillomatosis, polypoid cholangiocarcinoma and hepatocellular carcinoma with intraductal growth. Surgery was performed with peroperative histology of frozen samples showing papillary carcinoma. Paraffin embedded samples showed dysplastic epithelium of the bile ducts with diffuse papillary proliferations. There are atypical columnar cells and only slight development of fibrovascular structures. The epithelium shows ulcerations and a high grade of dysplasia with hyperchromatic nuclei and a large number of mitotic figures.
